摘 要:目的探讨多层螺旋CT(MSCT)后处理技术在眶骨骨折中的临床应用价值。方法收集65例眼外伤眶骨骨折患者的多层螺旋CT图像,将多平面重组图像与CT轴位图像进行比较,将后处理图像可以发现新病变、进一步证实CT轴位图像的可疑病变、没有新发现三种情况定为A、B、C级。结果A级28处(24例),B级51处(32例),C级106处(42例)。CT轴位图像检出106处,后处理图像检出185处。眶内壁骨折49例、外壁骨折23例、上壁骨折13例、下壁骨折21例;视神经管骨折8例;鼻泪管骨折1例。结论MSCT后处理图像可以多方位准确显示眶骨各部的骨折,并能够显示CT轴位图像不易发现及分辨不清的骨折。Objective To explore the clinical value of post-processing techniques of multi-slice spiral CT(MSCT) in orbital fracture. Methods Data of 65 patients with orbital fractures subjected to MSCT reformatted images and CT axial images were retrospectively reviewed. The patients were classified into 3 groups based on the utility of the advanced MSCT reformatted images : the grade A group ( substantial new information was obtained), the grade B group ( improved visualization and understanding of the orbital fracture were obtained), and the grade C group (no added useful information was obtained). Results There were 28 fractures (24 cases) in grade A group, 51 fractures (32 cases) in grade B group, and 106 fractures (42 cases) in grade C group. 106 fractures were shown by CT axial images, and 185 fractures by MSCT reformatted images. Fractures were located in the medial wall of the orbit in 49 cases, in the side wall of the orbit in 23 cases, in the superior wall of the orbit in 13 cases, in the inferior wall of the orbit in 21 cases, in the optic canal in 8 cases, and in the nasolacrimal duct in 1 case. Conclusion MSCT reformatted images can clearly and multidirectionally reveal the whole osteoporosis structure of the orbital fracture and provide more useful information nor presented in CT axial images.
