细胞凋亡和凋亡相关蛋白Fas在多囊肝病发病中的临床意义

摘　　要：目的观察细胞凋亡和凋亡相关蛋白Fas在多囊肝病发病中的临床意义。方法对43例多囊肝病患者囊壁肝组织和周围正常肝组织分别采用TdT介导的dUTP缺口末端标记技术(TUNEL)观察标本细胞凋亡,免疫组织化学方法检测Fas表达并结合临床资料进行分析。结果TUNEL显示囊肿壁肝细胞的凋亡指数(AI)明显高于正常组织。Fas在囊肿壁肝细胞中的表达明显高于周围正常组织。相关性分析表明,囊肿壁肝细胞AI与Fas的表达呈正相关。TUNEL表达阳性与否和年龄、性别、家族史、合并肝外囊肿、症状、血清白蛋白和谷丙转氨酶(GPT)水平无关;Fas的表达与合并肝外囊肿有关,但与年龄、性别、家族史、症状、血清白蛋白和GPT水平无关。结论细胞凋亡在多囊肝病的发生发展中起重要作用,肝细胞可通过高表达Fas诱导细胞凋亡,Fas的表达对判断多囊肝病患者是否合并肝外囊肿具有重要指导意义。Objective It is to observe the clinical significance of apoptosis and apoptosis-related protein Fas in pathogenesy of polycystic liver disease. Methods The specimen apoptosis in hepatic tissue of capsule wall and normal hepatic tissue around in 43 patients with polycystic liver disease was observed with TUNEL. The expression of Fas was detected immunohistochemistry. The data was analyzed with clinical data. Results TUNEL disclosed that apoptosis index （AI） in hepatic tissue of capsule wall was obviously higher than that in normal tissue. The expression of Fas in hepatic tissue of capsule wall was obviously higher than that in normal tissue around. Correlation analysis indicated that AI in hepatic tissue of capsule wall had positive correlation with the expression of Fas. The expression of TUNEL was male or not had no relation with age, gender, family history, combined with cyst outside liver, symptom and the levels of serum albumin and glutamic pyruvic transaminase （GPT）. The expression of Fas had relation with combined with cyst outside liver, but had no relation with age, gender, family history, symptom and the levels of serum albumin and GPT. Conclusion Apoptosis plays an important role in patho- genesy of polycystic liver disease. High expression of Fas can induce apoptosis of hepatocyte. Expression of Fas is important to indicate whether there is combined with cyst outside liver in polycystic liver disease.
