不同海况下模拟医院船开展手术的护理配合

摘　　要：目的探讨不同海况下模拟医院船船体摇摆度对手术配合、手术物品固定的影响程度,提出相应护理对策。方法将20只杂种犬随机分成4组,采用舰艇自由度操纵摇摆舱模拟0、2、5、8级4种海况,比较不同舰船摇摆度时犬颈外静脉留置针穿刺时间及手术器械固定情况。结果海况5、8级时,静脉留置针穿刺用时多于0、2级海况,差异有统计学意义(P<0.05);海况5、8级时,静脉留置针穿刺一次成功率低于0、2级海况,差异有统计学意义(P<0.05);海况5级以上、舰船横倾度≥8°时,操作人员姿势受影响,手术器械不易固定。结论了解各种海况下舰船中手术实施和术中护理配合的情况,熟练掌握摇摆中各种手术的手术步骤、加快输液操作的速度,并在手术中妥善固定手术器械,有助于医护人员充分预见实际海上手术操作难度,对提高工作效率和海上救护效果有重要意义。Objective To probe into the influence of ship sway on surgical nursing cooperation and fixation of surgical items under different sea conditions so as to propose the corresponding nursing measures. Methods Twenty dogs were randomly divided into four groups under the simulated four sea conditions （0,2,5,8 class） using freedom motion simulation cabin. The time of the dog＇s external jugular vein indwelling needle operations（EJVINO）the fixation of the surgical instruments under the 4 sea conditions were compared accordingly. Results The EJVINO time under 5 ~ 8 class sea conditions were significantly longer than those under 0 ＆ 2 class sea conditions（P（0.05）. The one-time success rates under 5 ＆ 8 class of sea conditions were significantly lower those under 0 ＆ 2 class of sea conditions （P〈0. 05）. The operators＇ positions were affected and the surgical instruments were hard to fix under the sea conditions over 5 with the heel level≥8°. Conclusion Understanding the surgical operation and operative coordination on ship under different sea conditions combined with good command of operative procedures, increased speed of transfusion practice and well-fixed surgical instruments contributes to foreseeable difficulties of surgical operations for the medical staff so as to increase their work efficiency and rescuing effect on sea.
