超声引导Mammotome旋切系统治疗乳腺良性病灶104例分析

摘　　要：目的探讨超声引导下Mammotome旋切系统在乳腺良性病灶的诊断和治疗中的应用价值。方法在超声实时监控下，利用Mammotome旋切系统对经超声检查诊断为良性的104例268个乳腺病灶行微创旋切治疗。结果268个乳腺病灶被微创切除；经病理检查，其中乳腺纤维腺瘤126处，乳腺囊性增生73处，乳腺腺病48处，乳腺腺病伴纤维腺瘤形成21处；术后并发局部血肿3例，无感染及气胸；术后超声提示无病灶残留，无1例严重并发症。结论超声引导下Mammotome微创系统治疗乳腺良性病灶，具有超声实时监控方便，定位准确，创伤小，美容效果好，并发症少，安全可靠等优点，是一种值得推广的手术方法。Objective To study the clinical value of uhrasound-guided Mammotome rotation system ap plied in the diagnosis and treatment of benign breast lesions. Methods Under real-time supervision of ultrasound,268 breast lesions in 104 patients diagnosed as benign breast lesions from ultrasound were treated with minimally invasive Mammotome rotation system. Results 268 cases of breast lesions were operated by the Mammotome system. According to the pathological investigation, there were 126 cases of fibroadenoma of breast,73 cases of cystic hyperplasia disease of breast, 48 cases of breast adenosis and 21 cases of breast adenosis with fibreademorea. After operations, local hematoma was found in 3 cases, but no infection and pneumothorax was found, no residual disease was detected by uhrasonography and no severe postoperative complication was found. Conclusions In the treatment of benign breast lesions, ultrasound-guided Mammotome rotation surgery is safe, reliable, minimal invasion, satisfactory cosmetic results and less postoperative complication. Real-time monitoring and accurate positioning are the two additional advantages of the technique. The technique deserves to be popularized.
