原发性体表软组织平滑肌肉瘤5例临床分析

摘　　要：目的分析原发性体表软组织平滑肌肉瘤的临床特征。方法回顾性分析2004年7月至2009年6月我院收治的5例体表原发性软组织平滑肌肉瘤患者的临床资料。结果5例患者中,2例为初发,2例为术后复发,1例为术后转移。2例初发病例行沿肿瘤包膜或假性包膜解剖去除加包括不多周围组织的局部边缘切除术,另3例则行将肿瘤及周围一定范围的正常组织一并切除术,1例术后接受局部放射治疗。所有病例术后恢复顺利,如期拆线治愈出院。5例患者随访1-34个月,除一例77岁患者因其他疾病术后1年去世,余4例均未发现复发及转移。结论原发性体表软组织平滑肌肉瘤是极少见的一种成人软组织恶性肿瘤,病灶切除或广泛切除是首选的治疗方法,复发转移可反复切除,不需行区域淋巴结清扫,术后应当辅以放疗和化疗。Objective To explore the characteristic of leiomyosareoma of the external soft tissue. Methods The clinical data of 5 patients with primary leiomyosareoma of the extemd soft tissue from July 2004 to June 2009 were analyzed and reviewed retrospectively. Results The clinical data include 2 cases of incipient, 2 cases of recurrence and 1 cases of metastasis postoperative. 2 cases of early onset of routine along the tumor capsule or pseudo -capsule anatomy plus the removal of surrounding tissue, including small local marginal excision, and another three cases of the tumor and the surrounding will be a range of normal tissue resection together, 1 case accepted local radiotherapy post - operation. Smooth postoperative recovery in all cases, patients were discharged on schedule. 5 cases had been followed up forl - 34 months, one case of 77 - year - old patients with other diseases to be died after 1 year. Recurrence and metastasis were not found in the other four cases. Condusion Leiomyosareoma of the external soft tissue is the extremely rare one kind of adult soft tissue malignant tumor. The first choice of treatment is to excise focus of infection or excise infection widespreadly. The recurrence and metastasis may excise repeatedly. Radiotherapy or chemotherapy is suggested postoperatively, but region lymph node cleaning is not needed.
