摘 要:目的为探讨损伤控制理念在特重型颅脑外伤救治中的应用价值,通过实验建立稳定的猪急性颅高压模型,为损伤控制的进一步研究奠定基础。方法选中型猪5只,采用微量泵注射自体动脉血方法形成脑内血肿,观察血肿形成过程中颅内压(intracranial pressure,ICP)、平均动脉压(mean arterial pressure,MAP)变化,2 h后行头颅CT扫描观察血肿情况,24 h后取脑行大体切片、苏木精-伊红染色光镜观察组织学病理改变。结果注血后2 h内出现明显的ICP、MAP上升,且ICP升至50 mmHg左右时出现呼吸、心率改变;头颅CT扫描显示脑内血肿形成满意。组织学显示血肿周围结构有破坏。结论采用微量泵注射自体动脉血方法可建立可靠的猪急性颅高压模型,可为损伤控制作进一步研究。Objective The purpose of this study was to investigate the clinical practicability of damage control surgery in the treatment of ultra-severe brain injury. Methods We established a model of acute increased intracranial pressure (ICP) in 5 pigs by injecting autologous arterial blood into the brain with the micro-pump syringe. ICP and mean arterial pressure (MAP) were monitored continuously from 2 hours before the blood injection to 2 hours after it, and CT scanning was performed 2 hours after the surgery. The brains were harvested at 24 hours for histological examination by HE staining. Results Within 2 hours after the blood injection, the model pigs showed significantly increased ICP and MAP and obvious changes in respiration and heart rate when ICP increased to about 50 mmHg. CT scanning and pathological examination revealed cerebral hematoma. Conclusion The porcine model of acute increased ICP can be successfully established by micro-pump injection of autologous arterial blood, which may help further studies of damage control neurosurgery.
