摘 要:目的:观察高强度聚焦超声(high intensity focused ultrasound,HIFU)对转移性肝癌患者机体免疫功能的影响。方法:转移性肝癌患者30例,通过全身麻醉、超声肿瘤定位进行HIFU治疗,采用流式细胞仪及双抗体夹心法,检测治疗前、后外周血T细胞亚群(CD3+、CD4+、CD8+)、NK细胞的百分率及sIL-2R的变化。采用LDH释放法检测NK细胞的杀伤活性。结果:HIFU治疗后CD3+、CD4+、NK细胞的百分率明显升高(P<0.05),CD8+细胞的百分率明显降低(P<0.05),NK细胞杀伤活性较治疗前升高,但无统计学差异(P>0.05);sIL-2R的水平较治疗前降低,但无统计学差异(P>0.05)。结论:HIFU具有免疫激活作用,可一定程度改善患者的机体免疫功能。Objective:To study the changes of the immune function in metastatic hepatic carcinoma patients after high intensity focused ultrasound treatment. Methods:Under general anaesthesia and ultrasound positioning, thirty metastatic hepatic carcinoma patients were treated by high intensity focused ultrasound. Flow cytometry (FCM) was used to detect the changes in percentages of T - lymphocyte subsets ( CD3+ , CD4+ , CD8+ ), NK cells and the contents of soluble interleukin-2 receptor (sIL- 2R) were detected by double sandwich ELISA before and after HIFU treat- ment. LDH enzyme -release assay was used to detect cytotoxic activity of NK cells. Results: After HIFU treatment, the percentages of CD3^+ and CD4^+ and NK cell:s significantly increased (P 〈 0.05 ), the percentage of CD8+ cells decreased markedly ( P 〈 0.05 ). By comparison with pre - therapy group, the cytotoxic activity of NK cells increased and the content of sIL- 2R decreased, but they both were no statistical difference between post -therapy and pre - therapy ( P 〉 0.05 ). Conclusion: High intensity focused ultrasound may activate and improve cell - mediated immune function in a certain degree.
