超声引导经皮肝胆管穿刺置管引流治疗梗阻性黄疸价值分析

摘　　要：目的探讨恶性梗阻性黄疸超声引导经皮肝胆管穿刺置管引流的方法及其治疗效果。方法87例恶性胆道梗阻患者分别采用超声引导经皮肝胆管穿刺置管引流（48例，观察组）和X线引导穿刺置管引流（39例，对照组），比较分析两组患者一次置管成功率、并发症发生率及黄疸消退情况。结果观察组一次置管成功45例（93．8％），发生并发症2例（412％）；对照组一次置管成功29例（74．3％），发生并发症8例（20．5％）。观察组一次置管成功率显著高于对照组（P〈0．05），而并发症发生率明显低于对照组（P〈0．05）。两组黄疸消退情况相似。结论采用超声引导经皮肝胆管穿刺置管引流是对恶性梗阻性黄疸的一种安全有效的姑息性治疗方法，可明显缓解黄疸．减轻患者痛苦，提高其生存质量。Objective Of malignant obstructive jaundice of ultrasound-guided percutaneous transhepatic puncture and catheter drainage method and therapeutic effect. Methods 87 cases of malignant biliary obstruction were used in patients with ultrasound-guided percutaneous transhepatic puncture and catheter drainage （48 cases, the observation group）, and X-ray guidance to puncture and catheter drainage （39 cases, the control group）, a comparative analysis of two groups of patients with a successful catheter rate, complication rate and jaundice cases. Results The success of an indwelling catheter observation group 45 cases （93.8%）, complications occurred in 2 cases （4.2%）; control group, the success of an indwelling catheter in 29 cases （74.3%）, complications occurred in 8 cases （20.5%）. The observation group a success rate of catheterization were significantly higher （P〈0.05）, while the complication rate was significantly lower than the control group （P〈0.05）. The two groups was similar to jaundice. Conclusion Use of ultrasound-guided percutaneous transhepatic puncture and catheter drainage of malignant obstructive jaundice is a safe and effective palliative treatment can significantly alleviate the jaundice, to reduce patient suffering, improve their quality of life.malignant obstructive jaundice is a safe and effective palliative treatment can significantly alleviate the jaundice, to relieve pain and improve quality of life.
