白内障复明手术在基层防盲中的应用

摘　　要：【目的】评价超声乳化术在内白内障防盲手术中的临床应用。【方法】采用低收费方式对湖南省内5625例贫困白内障患者进行扶贫手术，其中5023例行白内障超声乳化摘除联合人工晶体植入术；602例行小切口囊外联合人工晶体植入术，对其疗效进行分析。【结果】5625例白内障患者术后d1的脱盲率为99．6％（5601／5625），脱残率为86．9％（4889／5625）。术中术后主要并发症为角膜水肿，色素层装，后囊膜破裂，玻璃体溢出，高眼压，眼内炎。【结论】超声乳化术安全、快速、有效，在防盲手术中有明显的优越性，扩大了手术适应证；可以在一定程度上有效解决白内障致盲的问题。[Objective]To evaluate the application of phacoemulsification in the blinding-prevention operation of cataract. [Methods]A total of 5625 cases of cataract in Hunan Province were operated with lower charge, among which 5023 patients underwent phacoemulsification and intraocular lens implantation（IOL） and 602 underwent extracapsular cataract extraction（ECCE） plus IOL. [Results]The out of blindness rate and disability rate at the first day after operation were 99.6% and 86.9%, respectively. The main complications were edema of the cornea, uveitis, rupture of the posterior capsular, overflowing of the vitreous,high intraocular pressure（IOP） and endophthalmitis. [Conclusion]The phacoemulsification is safe, rapid and effective to resolve the blindness caused by cataract to some degree. And it expands the indication of operation with the significant superiority in blinding-prevention.
