巨大型颈静脉球瘤的诊断和治疗

摘　　要：目的探讨巨大型颈静脉球瘤的诊断和治疗经验。方法对７例病人行临床影像学检查和手术治疗，３例术后辅以放疗和／或γ刀治疗。结果颈静脉球瘤病人症状多不典型。肿瘤全切除４例，次全或大部切除３例，死亡１例。６例随访１～３５年，预后良好。结论要重视本病发病隐蔽的特点，及时进行影像学检查。Objective To research the diagnosis and treatment for patients with giant glomus jugular tumor (GJT). Methods Seven patients with giant GJT were undergone clinical examinations, imaging work-up and surgical operations. Postoperatively 3 patients received radio therapy and/or gamma knife treatment. Results The clinical manifestations of GJT were often not typical. Total tumor removal was achieved in 4 patients, sub-or large removal in 3. Postoperatively 1 patient died of pneumonia. Six patients followed up from 1-3.5 years had enjoyed a favorable outcome. Conclusions The concealed characteristics of GJT are required more careful and detail clinical and imaging examinations. Extensive posterolateral sub-occipital approach is helpful for total tumor resection and enhancement of the therapeutic effects.
