摘　　要：报告1例黑素瘤并发晕痣样色素减退斑。患者女,55岁。因左臀部黑色丘疹30余年,出现结节并逐渐增大和色素减退斑2年,破溃、结痂10个月就诊。皮肤科检查:左臀部一2 cm×3 cm×0.6 cm暗红色结节,表面破溃、结痂;结节周同绕以1～2cm宽、边界清楚的色素脱失晕。皮损组织病理检查示真皮中上部境界清楚的肿瘤团块,由多数梭形细胞和部分上皮样细胞组成,免疫组化染色示S-100(+),HMB45(+)。诊断为黑素瘤并发晕痣样色素减退斑。A 55-year-old female presented with a 30-year history of nevus pigmentosa on the left buttock, a nodule developed gradually with depigmentation for 2 years, and ulcerated with crusts for 10 months. Dermatological examination showed a 2 cm ×3 cm×0.6 cm dark-red nodule with ulcer and crust which were surrounded by a circumscribed depigmentation halo, 1-2 cm in width. Histopathology demonstrated a mass of tumor in the dermis, mainly composed of spindle cells and some epithetioid cells. Immunohistochemical stain showed expression of S-100 and HMB-45. The diagnosis was established as malignant melanoma with halo nevus-like depigmentation.
