食管癌患者手术前后血清CYFRA21-1、SCC的变化及临床意义

摘　　要：目的探讨食管癌患者手术治疗前后血清细胞角蛋白19片断(CYFRA21-1)、鳞状细胞癌相关抗原(SCC)水平的变化及临床意义。方法分别以电发光免疫测定法(ECLIA)和微粒酶联免疫测定法(MEIA)检测37例食管癌患者手术前后血清CYFRA21-1和SCC含量及35名健康人血清CYFRA21-1和SCC含量。结果食管癌组术前血清CYFRA21-1、SCC水平高于正常健康人组,差异有统计学意义(P<0.01)。食管癌患者术前血清CYFRA21-1、SCC水平高于术后,差异有统计学意义(P<0.01)。食管癌患者有淋巴结转移组血清CYFRA21-1、SCC水平高于无淋巴结转移组,差异有统计学意义(P<0.01)。在随访中发生转移或复发者,血清CYFRA21-1、SCC指标又再次升高,与术后早期比较差异有统计学意义(P<0.01)。结论食管癌患者行根治性切除术后,血清CYFRA21-1、SCC水平显著降低,检测术后CYFRA21-1、SCC水平及其变化对观察食管癌患者疗效及判断预后有重要的意义。Objective To investigate the variation and clinical significance of CYFRA21-1,SCC levels in preoperative and postoperative esophageal cancer.Methods Serum levels of SCC,CYFRA21-1 of 37 cases of esophageal cancer patients have been tested by electrochemical luminescence immunoassay（ECLIA） and microparticle enzyme immunoassay（MEIA） in preoperative and postoperative esophageal cancer,these indicators also tested in 35 samples of healthy people.Results Serum levels of CYFRA21-1 and SCC in preoperative patients were significantly higher than those in healthy group（P〈0.01）.These indicators in preoperative and lymph node metastasis patients were significantly higher than those in postoperative and without lymph node metastasis patients（P〈0.01）.These indicators rose again in metastasis or relapses patients during follow-up,and there were statistically significant differences（P〈0.01）.Conclusion Serum levels of CYFRA21-1 and SCC descended obviously after the radical operations.Changes of serum contents of CYFRA21-1 and SCC after operations might have great effect on prognosis of esophageal cancer patients.
