摘 要:目的探讨细胞周期蛋白E(Cyclin E)、基质金属蛋白酶-9(MMP-9)、生存素(Survivin)及染色体同源丢失性磷酸酶和张力蛋白基因(PTEN)的表达与大肠癌临床病理特征的相关性,以期在分子水平上更准确地判断大肠癌患者的预后,寻找相应的基因治疗方法。方法采用免疫组织化学Envision两步法检测66例大肠癌组织标本中Cyclin E、MMP-9、Survivin及PTEN的表达情况,12份大肠健康黏膜组织作为健康对照。结果66例大肠癌中CyclinE、MMP-9、Survivin蛋白的表达阳性率分别为62%、76%、71%,明显高于正常大肠黏膜上皮的表达(8%、25%、0),两者比较差异有统计学意义(P<0.01);PTEN在大肠癌中的阳性表达率为53%,明显低于大肠健康黏膜中的表达(92%),两者比较差异有统计学意义(P<0.05)。Cyclin E的异常表达与大肠癌Dukes分期及淋巴结转移相关(P<0.05);MMP-9的表达与浸润深度、组织分化程度、Dukes分期及淋巴结转移均相关(P<0.05);Survivin的表达与Dukes分期及淋巴结转移相关(P<0.05);PTEN表达与大肠癌浸润深度、Dukes分期及淋巴结转移相关(P<0.05)。结论Cyclin E、MMP-9、Survivin及PTEN基因蛋白的表达与一些临床病理特征具有相关性,Cyclin E、MMP-9和Survivin基因的表达上调与PTEN基因表达下调可能在大肠癌的发生、发展进程中发挥协同作用,检测这4种基因蛋白对于评估大肠癌的生物学行为具有重要价值,有助于估计患者的预后,为临床治疗提供依据。Objective To explore the expression of Cyclin E, MMP-9, Survivin and PTEN, and their correlation with clinicopathological characteristics of colorectal carcinoma. Methods Two-step envision immunohistochemistry system was used to determine the expression of Cyelin E, MMP-9, Survivin and PTEN in 66 surgically resected specimens of colorectal neoplasms and 12 normal colorectal mucosa as control group. Results The positive expression rates of Cyclin E, MMP-9 and Survivin in 66 cases of colorectal neoplasms was 62%, 76% and 71%, compared significantly with those in normal coloretal mucosa (8%, 25%, 0). The positive expression rates of PTEN in colorectal neoplasms (53%) was significantly lower than that in normal coloretal mucosa (92%, P〈0.05). The abnormal expression of Cyclin E correlated well with Dukes stage and lymph node metastasis (P〈0.05), as did the MMP-9 expression with depth of infiltration, Dukes stage and lymph node metastasis (P〈0.05) , Survivin expression with Dukes stage and lymph node metastasis (P〈0.05), FFEN expression with depth of infiltration, Dukes stage and lymph node metastasis (P〈0.05). Conclusion Expressions of Cyclin E, MMP-9, Survivin and PTEN showed correlation with some cfinico- pathological characteristics of colorectal neoplasms. The up-regulation of Cyclin E, MMP-9 and Survivin expression and down-regulation of PTEN may produce synergistic effect in the carcinogenesis and development of colorectal earcinoma. Detection of these four genes protein can be important for evaluating the biological behavior of colorectal neoplasms, which may be valuable for prognosis estimation and providing a rationale basis for clinical therapy.
