超声刀与电刀行腹腔镜下直肠癌全直肠系膜切除术的对照研究

摘　　要：目的比较腹腔镜下直肠癌全直肠系膜切除术中使用超声刀和电刀的临床效果。方法56例直肠癌分为2组,腹腔镜下使用超声刀(Ⅰ组)行直肠癌全直肠系膜切除术28例,高频电刀组(Ⅱ组)28例,对两组手术相关指标、术后恢复指标、术后并发症等方面进行比较。结果超声刀组的平均出血量、手术时间低于高频电刀组,而清除淋巴结数目高于高频电刀组。结论超声刀行腹腔镜全直肠系膜切除术治疗直肠癌,具有精确切割、凝血可控制的功能,出血少,手术时间短,且清扫彻底等优点,可在临床推广应用。Objective To compare the therapeutic effect of ultrasonically activated scalpel with electricity scalpel in total mesorectal excision under laparoscopy. Methods 56 patients with rectal cancer were divided into two groups treated with ultrasonically activated scalpel （group 1） and electricity scalpel （group 2）. The indexes during operation and complications were observed. Results The average operation time, volume of blood loss of group 1 were less than that in group 2. The number of lymphade node dissection was more than that in group 2. Conclusions Ultrasonically activated scalpel has precise cutting, controlled coagulation, less operating time and volum of blood loss, and more dissection.
