MSCT引导下植入^(125)I粒子治疗盆腔恶性肿瘤22例分析

摘　　要：目的探讨MSCT引导下植入125I粒子治疗盆腔难治性恶性肿瘤的临床价值。方法22例盆腔难治性恶性肿瘤患者,在MSCT引导下经皮穿刺,按照放疗计划在肿瘤内及周围放置125I粒子,植入即刻及术后2～12个月复查观察粒子在瘤体内的分布、有无并发症及疗效。结果22例恶性肿瘤粒子顺利植入,其中2例2个病灶粒子放置层面与计划不符,3例5个病灶部分粒子放置间距不够,17例21个病灶粒子放置位置均与计划大致相符。治疗2～12个月后复查,14例恶性肿瘤范围较前缩小(63.6%),3例(13.6%)恶性肿瘤范围较前大致相仿,1例(4.6%)恶性肿瘤范围较前略增大;4例(18.2%))患者因多器官衰竭死亡。22例患者术后均未见其他严重并发症及相关放射损伤。结论MSCT引导下植入125I粒子治疗盆腔难治性恶性肿瘤是一种安全、方法简便、有效的微创治疗方法。Objective To investigate the effects of the CT guided implantation of the particle of 125I in treatment of pelvic refractory malignant tumors. Methods 22 patients with pelvic refractory malignant tumors were treated with percutaneous CT-guided implantation of 125I in and surround the tumors according to the radiation treatment planning. The distribution of particle of 125I in the tumors, complications and effects were observed immediately and after 2 ~ 12 months. Results The particle of 125I were successfully implanted in 22 patients with pelvic refractory malignant tumors, and the placed location of the particle of 12si was basically same to the plan in 21 lesion location of 17 cases. Placed level of the particle of 125I was not correspondent with the plan in 2 cases, placed spacing between particle of 125I was not enough in 5 lesion location of 3 cases. Reexamination at 2~ 12 months after treatments showed that the volume of malignant tumors were reduced in 14 cases（ 14/22,64% ）, the tumors were approximately quite in 3 eases（3/22,14% ）, and the volume of malignant tumors was slightly increased in 1 case. 4 patients died of multiple organ failure. No serious complications or related radiation injury were observed. Conclusions The CT guided implantation of the particle of 125I in treatment of pelvic refractory malignant tumors is a safe, simple and effective method.
