ANA、ENA、dsDNA抗体在自身免疫性疾病中的敏感性及相关性分析

摘　　要：目的探讨ANA、ENA、dsDNA三类自身抗体在自身免疫性疾病中的敏感性及相关性。方法收集疑似自身免疫性疾病的患者血清2319例,用间接免疫荧光法测定ANA、dsDNA,用免疫印迹法测定ENA,并对结果进行相关性分析。结果在2319例标本中,ANA共检出阳性728例,ENA检出阳性200例,dsDNA检出阳性80例。在728例ANA阳性标本中,ENA阳性199例,dsDNA阳性79例;在1591例ANA阴性标本中,ENA阳性1例,dsDNA阳性1例。在86例nRNP为阳性的标本中,颗粒型80例;在31例Sm.nRNP同为阳性的标本中,颗粒型29例;在9例Scl-70的阳性标本中,均质型7例。结论ANA、ENA、dsDNA三类自身抗体在自身免疫性疾病中的敏感性有显著的差异;ENA多肽抗体谱中的Sm.nRNP.Scl-70的阳性与ANA的荧光模式有一定的关联;在ANA阴性的标本中检出ENA和dsDNA阳性的可能性很小。Objective To study the sensitivity and correlation analysis on antinuclear antibodies （ANA）, extracted nuclear antigens（ENA） and double-stranded DNA （dsDNA） in autoimmune disease. Methods Z319 serum of suspected autoimmune disease patients were collected. ANA and dsDNA were detected with indirect immunofluorescence and ENA were detected by immunoblotting. Results There were 728, 200 and 80 cases positive in ANA, ENA and dsDNA, respectively. There were 199 and 79 cases positive in ENA and dsDNA, respectively, of 728 positive in ANA. Conclusions There were significant differences in the sensitivity of ANA, ENA and dsDNA in autoimmune disease.
