摘　　要：目的:探索符合临床药师查房要求、简便、合理、连续、有效的药历书写模式。方法:在临床查房期间借鉴国内外药历模式及临床病历,建立住院药历。结果:初步建立了较为实用、清晰、规范、符合查房需要的药历。结论:住院药历的书写模式反映患者用药的整个过程,建立规范化药历可促进临床合理用药,临床药师进一步研究规范化药历的书写及推广具有重要的意义。OBJECTIVE： To explore a simple,reasonable,continuous and effective writing model of medication history that tailored to the demands on clinical pharmacists＇ ward rounding.METHODS： The inpatient medication history was established in ward rounding taking the medication history both at home and abroad and the clinical history as references.RESULTS： A practical,clear,standard medication history tailored to ward rounding has been established preliminarily.CONCLUSION： The inpatient medication history reflects patients＇ entire medication process.Establishing standardized medication history can help promote clinical rational medication.It is of great importance for clinical pharmacists to further study the writing of standard medication history as well as its popularization.
