多层螺旋CT在胃癌术前分期诊断中的价值研究

摘　　要：目的评价多层螺旋CT(MSCT)检查在胃癌术前分期诊断中的价值。方法对87例经胃镜活检证实的胃癌患者行MSCT平扫和增强扫描,并由两名有经验的医师进行TNM分期,其中69例与其手术后病理结果进行对比。结果 MSCT对胃癌术前T、N、M及TNM总分期诊断准确率分别为76.8%、82.6%、100%和82.6%,均与手术病理相一致(Kappa值分别为0.644,0.705,1.000和0.750)。结论 MSCT对胃癌的诊断有其一定的优越性,可提高胃癌术前TNM分期的准确性,是胃癌术前准确分期的可靠方法,可以指导胃癌患者的治疗。Objective To evaluate the value of MSCT in diagnosis of gastric carcinoma. Methods 87 gastric carcinoma patients who had been proved by gastroscopy biopsy before surgery have been diagnosised with multispiral computed tomography（MSCT）. The results of MSCT were prospectively analyzed by two doctors separately, and 69 cases compared with pathological staging after surgery. Results In compared with pathological results, the accuracy of MSCT for T stage was 76.8% , for N stage was 82.6% , for M stage was 100% and for TNM stage was 82.6%. All of them were correlated with surgical appearances and pathological findings （ Kappa values were 0.644, 0. 705, 1. 000 and 0. 750）. Conclusions MSCT plays a unique role in diagnosis of gastric carcinoma before surgery. It can improve the accuracy of TNM staging of stomach cancer and is a credible means for the preoperative stage of gastric cancer, guide diagnosis and give treatment of gastric carcinoma.
