摘 要:目的了解宫颈癌及宫颈上皮内瘸变(CIN)患者人乳头瘸病毒(HPV)感染状况及其基因型的分布。方法应用导流杂交基因芯片技术(HybriMax)检测168例宫颈癌及CIN患者HPV21种基因型,并对HPV感染率及型则分布进行统计学分析。结果168例宫颈癌及CIN患者中共检出127例(75.6%)HPV感染,其中CINI,CINI,CINI和宫颈癌中HPV感染率分别是52.4%(22/42),71.1%(32/45),85.4%(41/48)和97.0%(32/33),差异有统计学意义(P〈0.01);在宫颈癌及CIN中共检出14种HPV亚型,其主要基因型及其感染率分别是HPV16(38.1%),58(10.1%)和52(6.0%);在CINⅠ,CINⅡ,CINⅢ和宫颈癌组中,两种以上HPV基因型感染率分别为2.4%(1/42),6.7%(3/45),10.4%(5/48)和24.2%(8/33)。组间比较差异有统计学显著性意义(P〈0.01);高危型HPV阳性率在各年龄组依次为84.4%,46.4%和77.6%,低危型HPV阳性率在各年龄组依次为6.7%,7.1%和6.ooA。结论宫颈癌及CIN中HPV感染的基因型以HPV-16,58和52为最常见。高危型HPV感染是子宫颈病变发生、发展的高危因素。多重HPV感染可能促进宫颈癌的发生。Objective To investigate human papilloma virus(HPV) prevalence and HPV genotypes distribution in cervical carcinoma and intraepithelial neoplasia. Methods 168 women for cervical carcinoma and intraepithelial neoplasia were selected for 21 genotypes HPV detecting by HybriMax. HPV infection rate and its genotyping distribution were analyzed. Results HPV infection was detected in 127 of 168 eases(75. 6%) with cervical carcinoma and intraepithelial neoplasia,HPV infection in cervical intraepithelial neoplasia grade Ⅰ(CIN Ⅰ ) ,CIN Ⅱ ,CIN Ⅲ and cervical carcinoma were 52.4% (22/42), 71.1% (32/45), 85.4% (41/48) and 97.0% (32/33) respectively,and there were statistical significance(P〈0. 01);14 types of HPV genotypes were identified in cervical carcinoma and intraepithelial neoplasia,and more common genotypes were HPV16(38.1%) ,58(10. 1%) and 52(6.0%). Two or more genotypes of HPV infection rate was was 2.4%(1/42),6.7% (3/45), 10. 4% (5/48) and 24.2% (8/33)respectively. Comparison between groups were significantly different (P〈0.01). The rates different age group of high risk HPV were 77.8% ,46.4% and 82.1% ,The rates different age group of low risk HPV were 6.7%, 7.1% and 6-0%. Conclusion The most common genotypes in cervical carcinoma and intraepithelial neoplasia are 16,58 and 52. High-risk HPV infection to cervical lesions and development of high-risk factors,Multiple HPV infection may promote the occurrence of cervical cancer.
