摘  要:目的研究多囊卵巢综合征(Polycystic ovarian syndrome,PCOS)患者血清中白细胞介素-1受体拮抗剂(IL-1Ra)、白细胞介素-1β(IL-1β)和白细胞介素-18(IL-18)水平,为阐明PCOS的发病机制提供基础。方法设POCS和健康对照两组,采用酶联免疫吸附法(Enzyme-linked immunosorbent assay,ELISA)或化学发光法(chemilumines-cent immunoassay)检测87例PCOS患者组以及85例年龄和体重指数(Bodymass index,BMI)相匹配的健康对照组血清IL-1Ra、IL-1β、IL-18、性激素水平以及内分泌代谢指标。结果1.PCOS组空腹胰岛素(Fasting insulin,FINS)、胰岛素抵抗指数(Homeostatic model assessment for insulin resistance,HOMA-IR)、睾酮(Testosterone,T)、黄体生成素(Lutein-izing hormone,LH)以及泌乳素(Prolactin,PRL)水平高于健康对照组(P<0.05~0.001),而卵泡刺激素(Follicle-stimu-lating hormone,FSH)水平低于健康对照组(P=0.007)。2.PCOS组血清的IL-1Ra、IL-1β和IL-18水平均显著高于健康对照组[PCOS组分别为:(275.3±55.1)pg/ml、(29.2±5.1)pg/ml和(258.6±39.8)pg/ml,健康对照组分别为:(167.2±50.3)pg/ml、(12.6±3.3)pg/ml和(143.1±22.4)pg/ml,P<0.001],而IL-1Ra/IL-1β比值显著低于健康对照组[PCOS组为9.6±1.5,健康对照组为13.3±1.3,P<0.001]。3.控制BMI影响后的偏相关分析显示,PCOS组血清IL-1Ra、IL-1β和IL-18水平与FINS、HOMA-IR呈正相关(r=0.278~0.463,P均<0.05),IL-1Ra/IL-1β比值与HO-MA-IR呈负相关(r=-0.293,P<0.05)。结论PCOS患者体内炎症因子水平增高、调控失衡。IR独立于肥胖,作为关键因素导致了PCOS的发生与发展。Objective To investigate the serum levels of interleukin-1 receptor antagonist (IL-1Ra),interleukin-1β (IL-1β) and interleukin-18 (IL-18) in patients with polycystic ovarian syndrome(PCOS),and to provide a basis for understanding the pathogenesis of PCOS. Methods PCOS group (n=87) and healthy control group (n=85) were included in this study,they were matched in the age and body mass index (BMI). Serum IL-1Ra,IL-1β,IL-18,sex hormone levels and metabolic parameters were measured by enzyme-linked immunosorbent assay (ELISA) or chemiluminescent immunoassay. Results Serum levels of fasting insulin (FINS),homeostatic model assessment for insulin resistance (HOMA-IR),testosterone (T),luteinizing hormone (LH) and prolactin (PRL) in PCOS group were significantly higher than those in healthy control group (P0.05~0.001),but the level of follicle-stimulating hormone (FSH) in PCOS group was lower than that of healthy control group (P=0.007). The serum levels of IL-1Ra,IL-1β,and IL-18 in PCOS group were significantly higher than those in healthy control group (PCOS group:275.3±55.1 pg/ml,29.2±5.1 pg/ml,and 258.6±39.8 pg/ml,respectively; healthy control group:167.2±50.3 pg/ml,12.6±3.3 pg/ml,and 143.1±22.4 pg/ml,respectively; P0.001),but the IL-1Ra/IL-1β ratio was significantly lower in PCOS group than that in healthy control group (PCOS group:9.6±1.5,healthy control group:13.3±1.3,P0.001). Serum IL-1Ra,IL-1β and IL-18 levels were positively correlated with FINS and HOMA-IR levels (r=0.278~0.463,P0.05),and the IL-1Ra/IL-1β ratio was negatively correlated with HOMA-IR level in PCOS group (r=-0.293,P0.05) after the influence of BMI was corrected in the covariance analysis. Conclusions There are higher levels and inordinate inflammatory factors in the PCOS patients. IR which separates from obesity plays a key role in the development and progression of PCOS.
