摘  要:目的:检测卵巢颗粒细胞瘤(granulosa cell tumor,GCT)组织中抗苗勒管激素(anti-mullerian hormone,AMH)的表达,并用它的表达水平及相关的统计学方法对卵巢GCT的组织学分级标准进行分析评价。方法:用免疫组织化学方法探讨AMH在40例卵巢GCT组织中的表达。结果:AMH在临床Ⅲ期病例中的阳性表达率明显低于其在临床Ⅰ期和Ⅱ期中的阳性表达率(P<0.05)。按李瑛氏组织学分级标准的结果中,AMH在高、中分化组的阳性表达率明显高于低分化组(P<0.01,P<0.05),而在PhilipJ标准组织学分级的结果中,AMH的阳性表达率在高、中、低分化组之间的差异无统计学意义(P>0.05)。李瑛氏组织学分级标准与临床分期有正相关关系(r=0.632,P<0.01),而PhilipJ组织学分级标准与临床分期无相关关系(P>0.05)。结论:李瑛氏组织学分级标准可用于病理学对卵巢GCT的预后评估。Objective: To detect the expression of AMH in ovarian granulosa cell tumor and to evaluate the criterion of histological classification with the expression of AMH and related statistical method. Methods: Expression of AMH was measured with immunohistochemistry in 40 cases of ovarian granulosa cell tumor. Results: The expression ratios of AMH in ovarian granulosa cell tumor decreased from stage Ⅰto stage Ⅲ, and the ratios of stage Ⅰ and stage Ⅱ were significantly higher than those of stage Ⅲ (P〈0.05). In the result of histological classification according to the criterion of Li Ying-shi's histological classification, the expression of AMH in well differentiation and moderate differentiation was significantly higher than that in poor differentiation (P〈0.01,P〈0.05), but in the result of histological classification according to the criterion of Philip J, the expression of AMH in different differentiated tissues was no significant difference (P〉0.05). The criterion of Li Ying-shi's histological classification was positively correlated with clinical stage (r=0.632, P〈0.01), but the criterion of Philip J's histological classification was not correlated with clinical stage (P〉0.05). Conclusion: The criterion of Li Yingshi's histological classification can be used to evaluate the prognosis of ovarian GCT in pathology.
