摘 要:目的探讨分化抑制因子-1(Id-1)、DPC4/Smad4和p21WAF1/CIP1蛋白及其mRNA在正常子宫内膜、子宫内膜增殖症和子宫内膜样腺癌(EA)组织中的表达及其临床病理学意义。方法采用免疫组织化学和原位分子杂交方法,检测30例正常子宫内膜组织、30例单纯性增生过长、30例复杂性增生伴不典型增生、72例EA组织中Id-1、Smad4和p21WAF1/CIP1蛋白及其mRNA的表达。结果Id-1、Smad4和p21WAF1/CIP1蛋白及三者mRNA在EA中的表达率分别为86.1%(62/72)、44.4%(32/72)、63.9%(46/72)、73.6%(53/72)、41.7%(30/72)和61.1%(44/72),Id-1和p21WAF1/CIP1阳性率明显高于正常和各型增生内膜;而Smad4则相反;Id-1蛋白及mRNA过表达与组织学分级、临床分期、浸润程度和淋巴结转移有关(P值均<0.05);Smad4和p21WAF1/CIP1蛋白及mRNA阳性表达与组织学分级、临床分期和浸润程度有关(P值均<0.05);Id-1、Smad4和p21WAF1/CIP1蛋白与相对应的mRNA呈正相关;Id-1与Smad4表达呈负相关。结论Id-1、Smad4和p21WAF1/CIP1蛋白及mRNA表达异常与EA的发生发展有关,有可能成为判断子宫内膜组织癌变和转移的重要指标。Objective To explore the relationship of the expression of Id-1,Smad4 and p21^WAF1/CIP1 protein and mRNA in the development of endometrioid adenocarcinoma.Methods The expression levels of Id-1,Smad4 and p21^WAF1/CIP1 proteins and mRNA in endometrioid adenocarcinoma tissues(n=72),endometrial hyperplasia tissues(n=60)and normal tissues(n=30)were examined by immunohistochemistry and in situ hybridization method.Results The positive rates of Id-1,Smad4 and p21^WAF1/CIP1 protein and mRNA in normal endometrium,endometrial hyperplasia,endometrial atypical hyperplasia and endometrioid adenocarcinoma were respectively 86.1%(62/72)、44.4%(32/72)、63.9%(46/72)、73.6%(53/72)、41.7%(30/72)and 61.1%(44/72).The positive rates of Id-1 and p21^WAF1/CIP1 protein and mRNA were high than that in normal endometrium and endometrial hyperplasia,the opposite of Smad4.The positive expression of Id-1 was related to the histological grade(P〈0.01),FIGO stage(P〈0.01),depth of invasion(P〈0.05)and lymph node metastasis(P〈0.05).The positive expression of Smad4 and p21^WAF1/CIP1 was related to the histological grade(P〈0.05),FIGO stage(P〈0.05)and depth of invasion(P〈0.05).The expression of Id-1 protein was positively correlated with its mRNA.The expression of Id-1 was negatively correlated with Smad4.Conclusion The expression of Id-1,Smad4 and p21^WAF1/CIP1 protein and mRNA associate with the development and progression of endometrioid adenocarcinoma,which may be a important biomarker to diagnose the early carcinogenesis of endometrioid adenocarcinoma.
