喉鳞状上皮病变中细胞周期抑制蛋白p16^(INK4a)的异常表达

摘　　要：目的探讨喉鳞状细胞癌、喉鳞状上皮不典型增生及喉慢性炎中p16INK4a蛋白表达水平的变化及意义。方法应用免疫组化方法检测86例喉鳞状上皮病变(喉鳞状细胞癌16例,喉上皮不典型增生48例,喉慢性炎22例)组织中p16INK4a蛋白的表达,进行半定量分析及统计学检验。结果16例喉鳞状细胞癌中,p16INK4a全部阳性表达;48例喉上皮内瘤变中,35例p16INK4a阳性表达,阳性率为72.92%,其中轻度不典型增生,中度不典型增生,重度不典型增生p16INK4a阳性表达率分别是33.3%(4/12)、75%(15/20)、100%(16/16),且三者间差异有显著性(P<0.05)。22例喉慢性炎中,p16INK4a阳性表达6例,阳性表达率为27.27%(6/22)。p16INK4a阳性蛋白在喉鳞状细胞癌、喉鳞状上皮不典型增生及喉慢性炎组织中表达水平依次降低,且差异具有显著性(P<0.05)。结论喉鳞状细胞癌、喉鳞状上皮不典型增生中p16INK4a阳性表达明显高于喉慢性炎。p16INK4a在喉鳞状细胞癌、喉鳞状上皮不典型增生中过表达,提示其在喉鳞状细胞癌、喉鳞状上皮不典型增生发生发展中起重要作用。Objective To study the expression and significance ofp16INK4aprotein in throat squamous carcinoma, throat squamous cell atypia-hyperplasia and throat chronic laryngitis. Methods The expression of p16INK4a protein was detected in 86 throat samples （throat squamous carcinoma 16 cases, atypia-hyperplasia 48 cases, throat chronic laryngitis 22 cases） by immunohistochemieal staining. Results p16INK4a protein was positive in all 16 cases of throat squamous carcinoma. The positive expression of p16INK4a was detected in 35 of 48 cases of throat squamous cell atypia-hyperplasia with positive rate of 72.92%, in which 33.3% （4/12） was positive in low-grade atypia-hyperplasia, 75% （15/20） in middle-grade atypia-hyperplasia, 100% （16/16） in high-grade atypia-hyperplasia with significant difference among the 3 groups （P〈0.05）. The positive expression of p16INK4a was detected in 6 of 22 cases of chronic laryngitis with positive rate of 27.27%. p16INK4a increased with an increasing order in laryngitis, atypia-hyperplasia to squamous carcinoma with significant difference （P〈0.05）. Conclusion p16INK4a expresses significantly higher in throat squamous carcinoma, throat squamous cell atypia-hyperplasia than in throat chronic laryngitis. p16INK4a overexpresses in throat squamous carcinoma and throat squamous cell atypia-hyperplasia, which plays an important role in development of throat squamous carcinoma and throat squamous cell atypia-hyperplasia.
