地震致颌面部陈旧性骨折回顾性研究

摘　　要：目的回顾地震所致颌面部陈旧性骨折的治疗，为灾后颌面部损伤救治提供指导。方法32例地震所致颌面部陈旧性骨折患者均经过术前综合评估，颧骨、颧弓骨折组行颧骨颧弓切开复位内固定术，或者行颧骨增高或降低术；上、下颌骨骨折组行截骨及复位钛板固定术：眶底骨折组行眶底钛网植入；颞颌关节脱位组削低关节结节，截断喙突；其中上、下颌骨骨折组和颞颌关节脱位组均术后行颌问栓丝固定10天-2周。结果术后随访3～6月，所有伤员咬合及张口程度较术前明显改善，外形恢复满意，眶底骨折组术后无复视。结论按照现代正颌外科原理及方法治疗颌面部陈旧性骨折是目前较理想的方法。Objective To review the earthquake induced old fracture of maxillofacial treatment for post - disaster treatment of maxillofacial injuries to provide guidance. Methods 32 cases of the earthquake induced maxillofacial surgery patients have to undergo a comprehensive pre - assessment, zygomatic and zygomatie arch group with open reduction and internal fixation, or the line of higher or lower cheekbone surgery; The maxilla and mandible fracture group with osteotomy and reattachment titanium plate fixation ; Orbital floor fracture group titanium mesh orbital floor implant ; Temporomandibular joint dislocation group shaved articular nodules, truncated coracoid process ; Of these maxilla and mandible frac- ture group and the temporomandibular joint dislocation groups with plug wire intermaxillary fixation for 10 days to 2 weeks. Results Followed up 3 to 6 months, all the injured occlusion and mouth opening improved significantly comPared with the preoperative levels, the appearance was satis- factory,orbital floor fracture group no postoperative diplopia. Conclusion According to morden surgical orthodontics principles and methods to treatment maxillofacial old fractures is a more ideal methods.
