摘　　要：目的探讨术中近距离放疗对改善低位直肠癌局部控制率的意义。方法对ＤｕｄｅｓＣ期低位直肠癌采用Ｍｉｌｅ’ｓ手术加术中高危复发区或瘤床置管后装放射，６～１０Ｇｙ／次×２和术后盆腔外放射，４０～５８Ｇｙ／４～６周。结果１２例置管３１根，置管治疗成功率９３．５％（２９／３１），中位随访２０个月，置管放射区内无１例复发，４例治疗相关并发症均发生在内、外放射剂量偏高病人。结论本文提示术中置管后装治疗不受解剖部位限制，操作方便且可进行分割放射。Objecfive To explore the role of introperative Brachytherapy on the rectal carcinoma.Methods 12 patients with low rectal carcinoma were treated with Mile's operation and introperative brachytherapy combined postoperative external irradiation the brachytherapy was carried by 192 Iridium of HDR in the 2nd and 5th operative day, with the dose of 6-10Gy per time and an external irradiation was followed with the dose of 40 to 58Gy.Results The successful rate of putting the vector tubes was 93.5%. NO local recurrences were found in the region of brachytherapy, 4 complications (33.3%) connected with irradiation (2 the wound healed delayedly; 1 cystitis and 1 incomplete intestinal obstruction) occurred. Conclusion Introperative brachytherapy can be used as an adjuvant treatment methed of the Mile's operation for low carcinoma of the rectum
