急性脑梗死患者血浆同型半胱氨酸、内皮素、一氧化氮、叶酸、维生素B12变化及临床意义  

Clinical significance of the changes of plasma homocysteine, endothelin, mononitrogen monoxide, folic acid and vitamine B12 in acute cerebral infarction

摘　　要：目的探讨急性脑梗死患者血浆同型半胱氨酸（Hey）、内皮素（ET）、一氧化氮（NO）、叶酸（FA）、维生素B12（VitB12）变化及临床意义。方法测定140例急性脑梗死患者及61例健康对照者的血浆Hey、ET、NO、FA、VitB12含量。结果急性脑梗死组血浆Hey[（17．15±4．63）μmol／L]、ET[（32．62±18．76）μmol／L）显著高于对照组[（10．12±2．62）μmol／L、（20．08±13．38）μmol／L]（P〈0．01）；NO[（39．82±7．26）μmol／L]、FA[（7．54±4．01）ng／ml]．、VitB12[（247．70±119．85）ng／ml]水平低于对照组[（60．22±6．28）μmol,／L、（10．08±3．86）ng／ml、（411．52±194．58）mg／L]（P〈0．01）；高血压脑梗死患者Hey水平高于单纯脑梗死患者（P〈0．01）。结论急性脑梗死患者血浆Hey、ET浓度升高，NO、FA、VitB12的水平降低，且Hey水平与ET呈正相关，与NO、FA、VitB12呈负相关。Objective To detect the fluctuations of homocysteine（Hcy） ,endothelin（ET） ,mononitrogen monoxide（NO） ,folic acid（FA） and vitamine B12 （VitB12） in the plasma of patients suffered from acute cerebral infarction and to investigate the clinical sig- nificance. Methods 140 blood specimens from cerebral infarction patients and 61 specimens from healthy individuals were obtained in this study and a series of items of Hcy, ET, NO, FA and VitB12 were detected. Results The Hcy [ （ 17. 15 ±4. 63 ） μmol/L ] and ET [ （32. 62 ± 18.76） νmol/L] were increased in acute cerebral infarction（ACI） group compared with that in control group[ （ 10. 12 ± 2. 62） μmol/L, （20.08 ± 13.38） μmol/L respectively ] and the difference was significant （ P 〈 0.01 ）. The levels of NO [ （ 39. 82 ± 7.26） μmol/L], FA[ （7.54 ± 4.01 ） ng/ml ] and VitB,2 [ （ 247.70 ± 119. 85 ） pg/ml ] were decreased compared with control group [ （ 60. 22 ± 6.28） p, mol/L, （ 10. 08 ± 3.86） ng/ml, （411.52 ± 194. 58） pg/ml respectively ] and the differences significant （ P 〈0. 01 ）. Gender and hypertension affected the values of Hcy. Conclusions Hcy and ET were increased while NO, FA and VitB12 were decreased in acute cerebral infarction. Hcy had positive correlation with ET and negative correlation with FA and VitB12.
