保留膀胱颈环状肌和前列腺侧旁神经血管束的前列腺癌根治术

摘　　要：为减少或避免前列腺癌根治术后尿失禁和阳萎的发生，采用保留膀胱颈环状纤维和前列腺侧旁神经血管束的方法行前列腺癌根治术１３例，结果１３例术后无尿失禁９例，轻度可复性尿失禁４例；保留性神经手术８例，术后有性功能７例；术前内分泌疗法９例，术后病理均可见肿瘤细胞退变，萎缩改变。结论：前列腺癌膀胱颈无浸润者适宜作本术式。由于术后保留括约肌功能，尿失禁发生率低。因此。In order to lessen and avoid postoperative urinary incontinence and impotence,radical retropubic prostatectomy with preservation of the bladder neck circle fibre and the paraprostatic neurovascular bundles was conducted for 13 patients.Postoperative continence has been perfect in 9 and slight recoverable incontinence in the other 4.With nerve sparing operation,sexual function has been preserved in 7 of the 8 patients.In 9 patients with preoperative endocrine therapy,degeneration of the tumor cells has been noted in all.It was proposed that in prostatic carcinoma patients with no invasion to the bladder neck the above procedure is indicated especially in high risk patients.
