摘 要:目的探讨非小细胞肺癌中趋化因子受体CCR7的表达及其临床意义。方法采用免疫组化和RT-PCR法检测40例非小细胞肺癌中CCR7的表达(鳞癌23例,腺癌14例,腺鳞癌3例),阴性对照标本采用20例癌旁正常肺组织。结果免疫组化显示CCR7在25例(62.5%)非小细胞肺癌组织中表达和1例(5%)癌旁正常肺组织中表达,差异具有显著意义(P<0.01);RT-PCR显示CCR7 mRNA在29例(72.5%)非小细胞肺癌组织中表达和2例(10%)癌旁正常肺组织中表达,差异具有显著意义(P<0.01)。CCR7表达与肿瘤TNM分期、淋巴结转移、分化程度、局部浸润有关,但与年龄、性别、肿瘤大小、病理类型无关。免疫组化和RT-PCR法显示CCR7表达对肺癌淋巴结转移判断的敏感性、特异性、阳性预测值及阴性预测值分别为95.9%(100%)、77.8%(61.1%)、84%(75.9%)、93.3%(100%)。结论CCR7在非小细胞肺癌中高表达,且与淋巴结转移密切相关,是肺癌淋巴结转移的独立危险因素,CCR7的表达可作为预测NSCLC患者淋巴结转移的临床候选指标之一,对指导肺癌的靶向治疗具有一定价值。Objective: To investigate the expression level of chemokine receptor 7(CCR7) and explore its clinical value in non-small cell lung cancer.Methods: Immunohistochemistry was used to detect CCR7 protein and reverse transcription PCR(RT-PCR) was used to detect CCR7 mRNA in 40 cases of lung cancer(25 cases of squamous cell carcinoma,14 cases of adenocarcinoma,1 case of adenosquamous carcinoma).20 pericancerous normal lung specimens were as the negative control samples.Results: CCR7 protein expression was shown in 25(62.5%)non-small cell lung cancer specimens and 1(5%)case of normal lung specimens.There was a significant difference(χ2 =17.952,P0.01).CCR7 mRNA expression was showed in 29(72.5%) non-small cell lung cancer specimens and 2(10%)cases of normal lung specimens(χ2 =20.857,P0.01).The CCR7 expression was significantly associated with TNM stage、lymph node metastasis、differentiate degree and regional invasion,but not associated with age at surgery、gender、tumor size and pathological type.In the retrospectively study,the sensitivity,specificity,positive predictive value,negative predictive value of CCR7 expression for lymph node metastasis was 95.9%(100%)、77.8%(61.1%)、84%(75.9%)、93.3%(100%) respectively.Conclusion: CCR7 mRNA and protein are expressed highly in non-small cell lung cancer and with significant correlation to lymph node metastasis.CCR7 positive expression is an independent risk factor of lymph node metastasis in non-small cell lung cancer and can be a selected clinical reference marker to foresee lymphatic spread and has value in directing target therapy.
