急性时相反应对学龄前儿童维生素A与铁评价指标的影响

摘　　要：【目的】了解急性时相反应对学龄前儿童维生素A和铁状况评价指标的影响。【方法】检测385名学龄前儿童血清视黄醇、铁蛋白、血红蛋白以及C-反应蛋白(C-reaction protein,CRP)水平并分析指标间内在关系。【结果】不同CRP水平儿童的血红蛋白浓度无明显差异(P>0.05),CRP≥10 mg/L儿童血清视黄醇浓度明显低于CRP<10 mg/L儿童,铁蛋白则相反(P<0.05)。CRP≥10 mg/L儿童维生素A不足发生率显著高于CRP<10 mg/L儿童。调查对象维生素A不足发生率为34.03%,但1.56%儿童伴感染;铁储备正常率为53.25%,但3.12%儿童伴感染。【结论】急性时相反应对维生素A和铁等营养评价指标有明显影响,人群营养评价应同时考虑感染的影响。[Objective] To estimate the effect of the acute phase response on indicators of vitamin A and iron in preschool children, [Methods] A total of 385 preschool children were selected to measure the concentration of serum reti- nol, serum ferritin, hemoglobin and C-reactive protein （CRP）. [Results] There was no significant difference of the hemoglobin status between the subjects with normal and elevated CRP level. But the mean serum retinol concentration were significant lower in subjects with elevated CRP level than that in subjects with normal CRP level, while the status of ferritin made a contrary result to the status of retino[, especially the boys＇. Moreover the prevalence of inadequate vitamin A status in subject with elevated CRP level was obviously higbel thau that in subjects with normal vitamin A status （P^0.05）. In present study, the prevalence of inadequate vitamin A status was 34.04% with 1.56% subjects with elevated CRP level and the percent of normal iron storage was 53.25% with 3.12% subjects with elevated CRP level. [Conclusion] The acute phase response can significantly affect the concentrations of serum retinol and serum ferritin and the measurements of acute phase proteins will improve the assessment of micronutrient status.
