摘  要:目的研究乳腺癌血流能量图(angio)及微血管密度检测(microvessel density,MVD)与腋窝淋巴结转移的关系。方法利用Agilent sonos 5500、Acuson sequoia 512检测148例患者160个乳腺癌肿块内血流信号及血流能量图,并采用免疫组化法检测乳腺癌肿块内MVD,比较乳腺癌肿块内血流能量图及MVD与腋窝淋巴结转移的关系。结果有腋窝淋巴结转移组(LN+组)86个乳腺以2、3级血流为主,血流信号较无腋窝淋巴结转移组(LN-组)74个乳腺明显丰富(P<0.05)。LN+组MVD值大于LN-组(P<0.05)。乳腺癌组织MVD值随着Adler血流分级的增高而增大(P<0.05),随着组织学级别的增加而显著增大(P<0.05)。结论乳腺癌的血管生成活跃者,其腋窝淋巴结转移的可能性大。血流能量图检测与MVD检测结合,能很好地评估乳腺癌血管生成,对判断乳腺癌腋窝淋巴结是否转移有重要意义。Objective To research the relationships between tumor anglo and microvessel density (MVD) with metastasis of axillary lymph nodes in patients with primary breast carcinoma. Methods The present study involved 148 surgical patients with 160 cases of breast cancer. Agilent sonos 5500, Acuson sequoia 512 and immunohistochemistry were employed to determine the intratumoral blood flow signals (anglo test) and the MDV before ope-ration. The results obtained were compared with the incidence of axillary lymph node metastasis demonstrated by surgical biopsy. Results There were 86 lymph node positive (LN+) cases and 74 lymph node negative (LN-) cases in the series. The blood flow signals were more numerous (P〈0. 05) and the count of CD34 positive microvessels was higher (P〈0.05) in the LN+ group than in the LN- group, showing the presence of close relationship. With the increase in histologic grade of masses and adler grade of blood flow signals, MVD expression of tumor was higher (P〈0.05). Conclusions Joint use of anglo test and MVD can illustrate the angiogenesis of a primary breast carcinoma and be useful in distinguishing the probability of axillary lymph node metastasis.
