胃癌组织超声造影定量分析在胃癌肝转移中的价值

摘　　要：目的探讨口服胃窗超声造影联合静脉超声造影(超声双重造影)在评价胃癌肝转移中的价值。方法 141例经胃镜活检确诊胃癌患者行超声双重造影检查,所有病例按有无肝转移分为转移组(M1:22例)与非转移组(M0:119例),将两组间胃癌组织超声双重造影定量分析结果进行比较,分析各项参数与胃癌肝转移之间的关系。结果通过自动追踪对比定量分析(autotracking contrast quantification,ACQ),M1组与M0组相比到达时间(arrival time,AT)及达峰时间(time to peak,TTP)较快,AT及TTP差异无统计学意义(P>0.05),M1组峰值强度(peak intensity,PI)显著高于M0组[(25.61±3.33)dB对(21.51±4.14)dB,t=5.106,P=0.000],增强强度(enhanced intensity,EI)显著高于M0组[(24.80±3.58)dB对(19.26±5.65)dB,t=6.107,P=0.000],PI及EI显示差异有统计学意义(P<0.05)。结论超声双重造影定量分析可以作为预测胃癌肝转移的方法之一。Objective To investigate the value of oral and intravenous contrast-enhanced ultrasonography.（i.e.double contrast-enhanced ultrasonography DCUS） as a new method, to evaluate ,the gastric cancer in patients with hver metastasis using DCUS. Methods One hundred and forty one patients with gastric caner proved by endoscopio biopsy underwent DCUS examination and were enrolled in this study ,and they were divided into metastatic group （M1：22 cases） and non-metastatic group （M1： 119 cases） according to whether with the liver metastasis or not. The DCUS quantitative analysis results of these two groups were compared with each other. Results There was no significant difference in arrival time （AT） and time to peak （TTP） between M1 and M0 group （P〉0.05） . The peak intensity （PI） and enhanced intensity （EI） of the gastric cancer tissues in M1 group was higher than that in M0 group significantly[PI： （25.61±3, 33） dBvs. （21.51±4.14） dB, t=5.106, P=0.000; EI： （24.80±3.58） dB vs. （19.26±5.65） dB, t=6. 107, P=0. 000] analyzed by using autotracking contrast quantification （ACQ） software. Conclusions DCUS with quantitative analysis could be considered as a new method to predict liver metastasis in patients with gastric cancer.
