基质金属蛋白酶-2及其抑制因子在子宫颈上皮内瘤样病变进展中的作用

摘　　要：目的:观察基质金属蛋白酶-2(MMP-2)及其组织抑制物(TIMP-2)在CIN中的表达情况,探讨其与CIN的关系。方法:取CIN和柱状上皮异位组织标本51例,采用免疫组织化学SP法进行标记、Image-pro-plus5.0图像分析系统进行定量分析。结果:MMP-2表达在CINⅢ组高于柱状上皮异位组、CINⅠ组及CINⅡ组(P<0.05),在CINⅡ组高于柱状上皮异位组及CINⅠ组(P<0.05),柱状上皮异位组与CINⅠ组之间,差异无统计学意义(P>0.05)。TIMP-2的表达在CINⅢ组高于柱状上皮异位组、CINⅠ组及CINⅡ组(P<0.05),柱状上皮异位组、CINⅠ组及CINⅡ组各组间差异无统计学意义(P>0.05)。MMP-2/TIMP-2的比值,在CINⅢ组高于柱状上皮异位组及CINⅠ组(P<0.05),在CINⅡ组高于柱状上皮异位组(P<0.05),CINⅢ组与CINⅡ组、CINⅡ组与CINⅠ组、CINⅠ组与柱状上皮异位组比较,差异无统计学意义(P>0.05)。结论:在CIN中,随着CIN分级的升高,MMP-2、TIMP-2的表达水平上调,尤其是MMP-2/TIMP-2比率失衡与CIN的生物学行为密切相关,在判断CIN的潜在恶性程度、估计其预后中有一定意义。Objective： To observe the expression levels of matrix metalloproteinase - 2 （ MMP - 2 ） and its inhibition factor - tissue inhibitors of metalloproteinase （TIMP -2 ）, explore the relationship with cervical intraepithelial neoplasia （CIN） . Methods： 51 samples of CIN and columnar epithelium ectopy tissues were collected and analyzed quantificationally by immunohistochemical SP method and image - pro - plus 5.0 imaging analytic system. Results ： The expression level of MMP - 2 in CIN ]lI group was significantly higher than those in co- lumnar epithelium ectopy group, CIN I group and CIN l/group （ P 〈 0. 05 ）, the expression level of MMP - 2 in CIN I/group was significantly higher than those in columnar epithelium ectopy group and CIN Ⅰ group （ P 〈 0. 05 ）, there was no significant difference in expression level of MMP - 2 between colunmar epithelium ectopy group and CIN I group （ P 〉 0. 05 ） . The expression level of TIMP - 2 in CIN m group was significantly higher than those in columnar epithelium ectopy group, CIN Ⅰ group and CIN Ⅱ group （ P 〈 0. 05 ）, but there was no significant difference in expression level of TIMP - 2 among columnar epithelium ectopy group, CIN Ⅰ group and CIN H group （ P 〉 0. 05 ） . The ratio of MMP- 2/TIMP- 2＋in CIN Ⅲ group was significantly higher than those in columnar epithelium ectopy group, CIN Ⅰ group （P 〈0. 05 ）, the ratio of MMP - 2/TIMP - 2 in CIN Ⅱ group was significantly higher than that in columnar epithelium ectopy group （ P 〈 0. 05 ）, there was no significant difference in ratio of MMP - 2/TIMP - 2 between CIN Ⅱ group and CIN Ⅲ group, CIN Ⅰ group and CIN Ⅱ group, columnar epithelium ectopy group and CIN Ⅰgroup, respectively. Conclusion： Among the cases with CIN, the expression levels of MMP -2 and TIMP -2 increase with increasing grades of CIN, unbalance of MMP - 2/TIMP - 2 is related to biological behaviors of CIN closely, which plays an important role in evaluation of malignant degree an
