摘 要:目的探讨二维高频超声及彩色多普勒血流特征在乳腺癌诊断中的价值。方法回顾性总结108例113个乳腺病灶,112个经手术和病理证实的乳腺癌患者的高频彩超声像图特征,观察病灶的形态,边界,内部回声,钙化,后方衰减及彩色多普勒血流分布,血流程度情况,频谱多普勒表现。结果本组108例患者共113个病灶,超声诊断乳腺癌80个,癌可疑17个,误诊12个,漏诊4个,手术病理符合乳腺癌者112个,总诊断符合率约88.9%(96/108)。彩色多普勒检查结果,病灶内部及周边见血流信号者53个,穿入血流4个。频谱多普勒检查PSV平均为:0.11±0.08m/s,RI平均为:0.87±0.09。结论二维高频声像图结合彩色多普勒检查,能有效提高乳腺癌的诊断正确率。Objective To investigate the value of high frequency color Doppler ultrasonography in the diagnosis of breast cancers. Methods A total of 108 patients with 113 breast tumors confirmed by operation and pathology were examined with high frequency color Doppler ultrasonography. The features including tumor shape, margin, inner echngenicity, calcification, acoustic attenuation, and color flow signals and Doppler waveform on color Doppler ultrasonography were analyzed retrospectively. Results Among t 13 breast tumors in 108 patients, ultrasonography diagnosed 80 tumors as breast cancer, suspected 17 tumors as cancer, misdiagnosed 12 tumors and missed 4 tumors preoperatively. The diagnostic accuracy for ultrasonography was 88.9% (96/108). Color Doppler ultrasonography showed color flow signals inside and around the lesions in 53 tumors and penetrating flow signals in 4 tumors. The peak systolic velocity was 0.11 ± 0.08m/s, and RI was 0.87 ± 0.09. Conclusion High frequency gray scale and color Doppler ultrasonography are valuable in improving the diagnostic accuracy for breast cancers.
