摘 要:目的:了解脑出血血肿抽吸术在我国的应用情况。方法:通过检索我国4个医学文献数据库获取相关文献,根据血肿抽吸术术式的不同分为硬通道抽吸术(HTAC)类和软通道抽吸术(STAC)类,分析研究的地域分布、执行的医院等级、时间、手术时间窗、血肿部位、大小及手术方法。结果:共获得合格文献1205篇(72855例),其中HTAC文献851篇(49521例),STAC文献354篇(23334例);文献遍及全国31个省的各级医院;HTAC在基层医院和由神经内科医师比STAC开展更多(P<0.001)。病例年龄在2~97岁。文献报道在90年代末逐年递增;HTAC增加幅度比STAC更显著(P<0.001)。手术多在时间窗6~72h内完成。手术血肿见于颅内任何部位,其中以幕上血肿最多(66.9%)。作为手术适应证的血肿大小≥30mL的研究最多(55.9%)。HTAC手术方法高度一致,STAC却相差较大。结论:血肿抽吸术在我国得到广泛开展,近年发展迅速;手术适应证范围广。Objective: To systematically review the application of the aspiration of intracerebral clots in treatment of intracerebral hemorrhage. Methods: The relevant reports were searched from four major Chinese medical databases. All studies were assigned into two groups of hard or soft tunnel aspiration of clots (HTAC or STAC). The data including geographical distribution, hospital hierarchy, time, surgery time window, the location and size of hematomas and surgical methods were analyzed. Results. Total 1205 papers were obtained with the authors coming from different type of hospitals in 31 provinces in the mainland of China. The numbers of HTAC and STAC papers were 851 (49521 patients) and 354 (23334 patients) respectively. The HTAC were performed preferentially by neurologists and in primary hospitals compared with STAC (P〈0. 001). In the end of 90s, the amount of reported papers is increased gradually with a more significant increase of HTAC (P〈0. 001). The surgeries were mostly performed within 6--72 h after onset. Hematomas were in any part of brain, but most (66.9%) were supratentorial. Most studies (55.9%) recruited the patients with hematoma volume ≥30 mL. The details of HTAC manipulation were highly consistent whereas the operation of STAC were variable. Con clusion.. Aspiration of clots in treatment of ICH has been extensively performed, especially in recent years. The indication for aspiration is extensive.
