摘 要:目的:用海水浸泡失血性休克犬模型,观察高渗盐经骨髓腔通路复苏休克犬的血流动力学变化。方法:将海水浸泡失血性休克犬随机分为对照组(Sham组)和实验组,实验组分为生理盐水(NS)组和高渗氯化钠经乙基淀粉40溶液(HSH)组,监测犬基础状态及复苏后各时间点的平均动脉血压(MAP)、中心静脉压(CVP)、髓内压及外周血钠、乳酸水平和中性粒细胞(PMN)数量。结果:实验组MAP、CVP、髓内压与Sham组相比均显著下降(P<0.05),乳酸水平、PMN数量显著上升(P<0.05)。结论:海水浸泡失血性休克犬时经骨髓腔通路输注HSH,可以很快稳定血液动力学指标,改善组织器官灌注和氧合,防止炎性细胞的附壁、游出,并可减轻组织器官损伤。Objective. To investigate the hemodynamic changes in experimental sea water immersion treated hemorrhagic shock model dogs, which were administrated with infusion of hypertonic brine through channel of bone marrow cavity. Methods: The sea water immersion treated model dogs were randomly divided into control group (Sham group) and experimental group. The latter was then randomly di- vided into two subgroups, of which one was treated with normal saline (NS group), and the other was treated with hypertonic saline solution by the 40-ethyl starch (HSH group). The average arterial blood pressure (MAP), central venous pressure (CVP), intramedullary pressure, the blood sodium, lactate and concentration of neutrophil (PMN) of the dogs were measured at baseline and several time points after resuscitation. Results:Compared with the Sham group, the MAP, CVP and intramedullary pressure of dogs in the experimental group had significantly decreased (P〈0.05), while the lactate and PMN increased significantly (P〈0.05). Conclusion: Infusion of hypertonic brine through bone marrow cavity channel may quickly stabilize hemodynamic indicators and improve perfusion and oxygenation of tissues and organs. It can also alleviate tissues and organs damage through prevention of the attachment or swimming out of inflammatory cells.
