血红蛋白浓度与鼻咽癌放射治疗效果分析

摘　　要：目的:探讨鼻咽癌(NPC)患者的血红蛋白浓度(HBC)对放射治疗疗效的影响。方法:回顾性分析2001年1月1日～2004年12月31日接受根治性放疗的NPC患者336例临床资料,根据放疗前HBC分为贫血组(男性<120g/L,女性<110g/L)和正常组(男性≥120g/L,女性≥110g/L),根据放疗前、后HBC变化情况分为HBC下降组(放疗结束时HBC<放疗前)和HBC未下降组(放疗结束时HBC≥放疗前)。采用电子直线加速器8MVX线进行常规分割外照射,比较各组间局控率和5年生存率。结果:贫血组和正常组的局控率差异无统计学意义(P>0.05),贫血组和正常组的5年生存率分别为35.12%和55.36%,两组比较差异有统计学意义(P<0.01)。正常组中放疗后HBC下降组的5年生存率较未下降组低,但差异无统计学意义(P>0.05),贫血组中放疗后HBC下降组的5年生存率较未下降组明显降低,差异有统计学意义(P<0.01)。结论:放疗前HBC可影响鼻咽癌患者放射治疗的局控率和5年生存率。Objective. To investigate the correlation between hemoglobin concentration （HBC） and the radiotherapy efficiency in patients with nasopharyngeal carcinoma （NPC）. Methods： Data of 336 patients who were confirmed by histopathology examination as NPC and were admitted to our hospital during January 1, 2001 and December 31, 2004 was retrospectively analyzed. Cases that were shown as HBC 〈120 g/L for male and HBC 〈110 g/L for female were classified as anemia group, and the rest were normal group. Cases that were shown by comparison of the, HBC before and after radiotherapy as decrease were classified as HBC decrease group. Electronic linear accelerator 8MV X lines was used for conventional fractionation irradiation. Results： The 5-year survival rates of the anemia group and normal group were 35.12% and 55.36% respectively, showing significant difference between them （P〈0.01）, while the local control rates of the two groups were not significant different （P〉0.05）. Survival rates of patients in the HBC decrease group were lower than those who were not in this group, especially; the difference was significant among patients in the anemia group （P〈0. 01）. Conclusion. The pre-radiotherapy hemoglobin concentration is related to the local control rate and 5-year survival rate of nasopharyngeal carcinoma patients who were treated by radiotherapy.
