口内入路摘除颌下腺13例临床分析

摘　　要：目的:探讨口内入路颌下腺切除术的可行性、手术安全性及美容效果。方法:经鼻插管全麻下,对13例需行颌下腺摘除的患者,从口内自第1双尖牙近中至第2磨牙远中舌侧口底黏膜入路,分离解剖出与颌下腺有关的神经和血管,完整摘除颌下腺。结果:13例患者颌下腺均顺利摘除,切口愈合良好,口内瘢痕小、隐蔽。经平均6个月观察,局部无感染,未出现大出血、面神经下颌缘支、舌神经和舌下神经损伤等并发症。结论:从口内入路摘除颌下腺,颌面部皮肤无手术瘢痕,有效避免了面神经下颌缘支的损伤,获得较理想的美容效果,可代替传统颌下区入路的颌下腺摘除术。Objective： To investigate the feasibility, safety and the cosmetic outcomes of submandibular gland resection by intraoral approach. Methods： Under general anesthesia with nasal intubation, submandibular g arated through d ands of 13 cases were completely removed after related nerves and blood vessels were sepstal lingual mueosa which was spread from the medial surface of the first premolar to the distal surface of the second molar. Results： The submandibular glands of 13 patients were excised successfully with well healed incision and small intraoral scars. During an average of 6-months observation, neither local infection, bleeding, nor damages to mandibular marginal branch of facial nerve, lingual nerve or hypoglossal nerves were observed. Conclusion： With advantages of no surgery scars, avoiding damages to marginal mandibular branch of facial nerves, intraoral approach can replace the conventional submandibular area approach in submandibular glands excision.
