弥漫大B细胞性淋巴瘤survivin表达与其预后的关系

摘　　要：目的探讨弥漫大B细胞淋巴瘤survivin表达与临床表现的关系。方法收集有完整随访资料的弥漫大B细胞性淋巴瘤(diffuse large B-cell lymphoma,DLBCL)患者128人,均有5年以上完整的随访资料。用免疫组化SP法检测其survivin表达情况,并分析其表达与临床预后的关系。结果128例DLBCL患者中有84例survivin蛋白表达阳性,阳性率为65.6%。表达阳性者与阴性者5年(overall survival,OS)分别为58.8%和18.7%,差异具有统计学意义。在淋巴瘤国际预后指数(international prognostic index,IPI)积分低危患者中,survivin表达阳性者5年总生存期OS及无病生存期(disease free survival,DFS)明显低于表达阴性者;在国际预后指标(international prognosis index,IPI)积分高危患者中,survivin表达阳性与阴性者5年OS无明显差异。结论Survivin是DLBCL的一个较有价值的预后指标,与IPI联合应用可筛选出低危患者中预后不良病例。Objective The purpose of this study is to examine the expression of survivin in diffuse large B- cell lymphoma（DLBCL）. Methods 128 patients with DLBCL and complete follow-up information for more than five years were enrolled in this study. The expression of survivin was determined by immunohistochemical staining. The relationship between clinical features and prognosis were analyzed. Results 84 cases （65.6%）had positive expression of survivin. The patients with positive survivin expression had a poor 5-year overall survival （58.8%） than the survivin negative patients （18.7%）. For the patients with a low risk international prognostic index （IPI）, the 5 years overall survival is lower in the patients with positive survivin expression than the survivin negative patients. This observation was not found in patients with a high risk IPI. Conclusion Survivin is a valuable prognostic marker in low risk DLBCL patients.
