基质金属蛋白酶-7在甲状腺良恶性病变组织中的表达及其意义

摘　　要：目的探讨甲状腺良恶性病变组织中基质金属蛋白酶-7（MMP-7）的表达及其临床意义。方法应用微波-LSAB免疫组化法检测50例甲状腺癌、45例甲状腺腺瘤和20例癌旁甲状腺组织中MMP-7的表达。结果甲状腺癌中MMP-7表达阳性率（64．0％）显著高于甲状腺腺瘤（37．0％）和癌旁甲状腺组织（25．0％）（x^2=8．72，6．52，P〈0．01）。MMP-7表达阳性率与甲状腺癌组织类型无关；有淋巴结转移者显著高于无淋巴结转移者（P〈0．05）；病理分期Ⅲ～Ⅳ期病例显著高于Ⅰ-Ⅱ期病例（P〈0．05）。MMP-7阳性的甲状腺癌复发及病死率显著高于阴性者（P〈0．05）。结论MMP-7表达对甲状腺癌恶性程度判断、生物学行为预测和预后评估有一定价值。Objective To investigate the expression of matrix metalloproteinases-7 （MMP-7）in benign and malignant thyroid tissues and its clinical significance. Methods The expression of MMP-7 in 50 thyroid cancers, 45 adenoma and＇20 adjacent noncancerous portion tissues were studied by microwave- LSAB immunohistochemical method. Results Thepositive rate of MMP-7 in thyi＇oid cancer was 64. 0% , which was significantly higher than 37. 0% in adenoma and 25.0% in adjacent noncancerous portion（ X2 = 8. 72,6. 52, P 〈 0. 01 ）. No correlation was observed between the expression of MMP-7 and histological grading of thyroid cancer. The expression of MMP-7 in lymph node metastasis and Grade m - IV were both higher than that in negative cases and in Grade I - 1I （ P 〈 0. 05 ）. The percentage of recurrence and death in MMP-7 positive cases were notably higher than that in negative cases（ P 〈 0. 05 ）. Conclusion The expression of MMP-7 can be regarded as a parameter for evaluating the degree of malignancy, biological behavior and prognosis of thyroid cancer.
