摘  要:目的分析探讨hTERC基因在不同程度子宫颈病变脱落细胞中的表达及对早期宫颈病变筛查的意义。方法采用FISH检测120名正常子宫颈脱落细胞、86例子宫颈上皮CIN及14例子宫颈鳞状细胞癌脱落细胞hTERC的表达情况,并分析hTERC的表达与宫颈病变发展的相关性。结果hTERC基因在正常子宫颈脱落细胞、炎症或伴湿疣组宫颈上皮CINⅠ期、CINⅡ期、CINⅢ期及SCC组中的阳性表达率分别为1.7%(2/120)、2.6%(1/38)、55.6%(10/18)、87.5%(14/16)、100.0%(14/14)和100.0%(14/14)。hTERC基因阳性表达率在SCC/CINⅢ期与CINⅠ期/CINⅡ期的差异有统计学意义(P=0.01);CINⅠ~Ⅲ期组与健康对照组比较,其阳性表达率差异也具有统计学意义(χ2=113.52,P〈0.01);hTERC基因在CINⅢ期和SCC组的阳性表达率用直接概率检验,差异无统计学意义(P=1.00),但随着病变加重,SCC组多倍体的拷贝数显著增加(hTERC基因扩增杂交信号≥3:3的细胞数占总异常细胞数的比例在CINⅠ期、CINⅡ期、CINⅢ期和SCC组中分别为6.12%、7.98%、28.07%和33.97%),且呈渐进性上升趋势。结论hTERC基因在CIN和SCC中的表达异常,且随病变程度增加阳性表达率增加,hTERC基因可作为SCC早期筛查的生物遗传学监测指标。Objective To investigate the expression and significance of the human telomerase gene (hTERC) in the cytologic specimens of cervix for early cervical lesion screening. Methods Cytologic samples of cervix including 120 normal persons, 86 CIN patients and 14 SCC patients were analyzed for aberrations of 3q26 using a commercially available two-color FISH probe. Results Positive expression rates of hTERC gene in normal, CIN Ⅰ, CIN Ⅱ ,CINⅢ and SCC samples were 1.7% (2/120) ,2. 6% ( 1/38), 55.6% ( 10/18 ) ,87. 5% ( 14/16 ), 100. 0% (14/14), 100. 0% ( 14/14 ), respectively. Compared with CINⅠ/CINⅡ group, hTERC gene positive expressions in SCC/CINⅢ group were higher significantly (P = 0. 01 ) , and there were also difference between CINⅠ-CINⅢ group and normal controls (χ2 = 113.52 ,P 〈 0. 01 ). With progression from CIN to carcinoma, polyploidization resulting in aneuploidy and high TERC gene copy numbers in SCC group ascended significantly. Among the total abnormal cells, the number of the cells percentage which hTERC amplification signals was more than 3 : 3 in CIN Ⅰ , CIN Ⅱ , CINⅢ and SCC groups were 6. 12%, 7. 98%, 28.07% and 33.97%, respectively. Conclusion The hTERC gene is increasingly gained with progression of CIN and may appear to be another screening testmarker for early cervical cancer screening and accessory diagnosis.
