恶性瘤病人术前化疗后氧自由基及红细胞改变

摘　　要：目的了解恶性瘤病人术前化疗后氧自由基变化和红细胞形态及渗透脆性改变。方法对术前化疗与未化疗各２０例于术前进行过氧化脂质（ＬＰＯ）、黄嘌呤氧化酶（ＸＯ）及超氧物歧化酶（ＳＯＤ）检测，对两组各５例作红细胞透射和扫描电镜观察，对两组各１０例作红细胞渗透脆性试验。结果化疗组和非化疗组两组病人的ＬＰＯ、ＸＯ均明显高于正常值，而ＳＯＤ则明显低于正常值，化疗组更为突出。两组红细胞透射电镜和扫描电镜观察均见形态改变并以化疗组较为突出。化疗组病人红细胞抗低渗能力降低。术中化疗组病人比非化疗组病人血压波动大，出血多，术后苏醒慢。结论两组病人术前均存在氧自由基升高，并以术前化疗者更为严重，引起红细胞形态改变和抗低渗能力改变也更大，提示对术前化疗恶性瘤病人术中要注意加强监护。Objective To study the changes of oxygen free radical(OFR)and RBC shape changes in preoperative chemotherapy patients with malignant tumor.Methods The LPO,XO and SOD were assayed in each 20 patients with and without preoperative chemotherapy.The RBC from 5 cases each of the two groups were observed by TEM and SEM.The osmotic fragility of RBC from 10 cases each of the two groups patients of malignant tumor with antechemotherapy and from 10 normal subjects were measured.Results The values of LPO,XO of both groups were higher than normal and the abnormalities were more obvious in the preoperative chemotherapeutic group.During operation,more violent fluctuation of circulation,more amount of hemorrhage and more retarded postoperative consciousness were seen in those patients of the preoperative chemotherapeutic group.In addition,the RBC from both groups were changed morphologically under TEM and SEM observation.Increased osmotic fragility of RBC were seen in both groups and obvious in the chemotherapy group.Conclusion The above-mentioned changes were caused probably by the tumor and chemotherapy.Therefore the anti-OFR treatment should be given to these patients during anesthetic and operative periods.
