摘 要:目的:探讨氯膦酸二钠脂质体(liposomal clodronate)对重症急性胰腺炎(SAP)大鼠胰腺损伤的保护作用。方法:利用薄膜法制备氯膦酸二钠脂质体。采用胰腺被膜下均匀注射5%牛磺胆酸钠制作SAP模型。SD大鼠48只,随机分为对照组(C组)、空白脂质体组(P组)、氯膦酸二钠脂质体组(T组)。P组和T组大鼠制作SAP模型。制模2h和6h后取肠系膜上静脉血液,检测各组大鼠血清中淀粉酶、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的含量,观察各组胰腺组织的病理学变化及胰腺病理评分,免疫组化检测胰腺Caspase-3的表达。结果:P组大鼠的血清淀粉酚水平较C组明显升高(P<0.01),T组大鼠的血清淀粉酶水平显著低于P组。P组较C组2、6h血清IL-6、TNF-α明显升高(P<0.01),T组较P组大鼠的2、6h血清IL-6、TNF-α水平显著降低(P<0.01)。T组大鼠的胰腺病理变化较P组明显减轻,胰腺组织病理学评分明显降低(P<0.01)。胰腺Caspase-3阳性表达增多。结论:氯膦酸二钠脂质体可选择性清除单核/巨噬细胞,对SAP大鼠胰腺损伤有保护作用。Objective:To investigate the protective effect of lipsomal clodronate against pancreas injury in rats with severe acute pancreatitis(SAP).Methods:Lipsomal clodronate was prepared by means of thin film.The models of SAP were established by injection of sodium taurocholate into the pancreatic capsule.The SAP models were prepared and randomly assigned to the model control group(C),SAP+Liposome group(P),SAP+Liposomal Clodronate group(T).The serum levels of IL-6,TNF-α were measured,and the changes of amylase(AMS) in serum were determined.Caspase-3 expression was observed using immunohistochemitry.Pathologic alteration of pancrease were observed and the pathological score of pancreatic tissue was also estimated.Results:In P group,serum levels of AMS were significantly elevated compared with C group (P0.01).In T group,serum levels of AMS were significantly decreased as compared with P group (P0.01).In P group,serum levels of IL-6、TNF-α were significantly elevated as compared with C group (P0.01).In T group,serum levels of IL-6、TNF-α were significantly decreased as compared with P group (P0.01).In T group,pathologic damages of pancreas were significantly decreased as compared with P group and pancreatic tissue pathological score were lower than those in P group(P0.01).In T group,expression of the Caspase-3 were significantly elevated as compared with P group.Conclusion:Monocytes/macrophages can be depleted via phagocytosis of liposome encapsulated clodronate.Liposomal Clodronate could alleviate the pancreas injury in SAP rats.
