幽门螺杆菌babA2和胃黏膜上皮Le^b抗原在不同血型中表达情况及与消化性溃疡的关系

摘　　要：目的:研究不同血型(ABO)患者幽门螺杆菌(Helicobacter pylori,Hp)表达血型抗原结合黏附素(bloodgroup antigen binding adhesion,babA2)和胃黏膜上皮表达Lewisb(Leb)情况,及两者与消化性溃疡的关系。方法:测定56例(25例胃炎,31例消化性溃疡)Hp阳性消化性溃疡及消化不良的患者血型,并进行内镜检查,取2块胃黏膜,一块进行Hp培养,采用PCR检测HpbabA2表达情况;另一块组织进行组织病理学检查,并应用免疫组化测定胃黏膜上皮表达Leb情况。分别比较不同血型babA2、Leb表达情况与消化性溃疡的关系。结果:56例患者中,消化性溃疡组O型血所占比例高于胃炎组及正常人群(P均<0.0125),O型血患者胃黏膜Leb评分显著高于非O型血(P<0.005),消化性溃疡组Leb评分高于胃炎组(P<0.05)。Hp培养成功30例,有28例babA2阳性(7例消化性溃疡,21例胃炎),babA2阳性患者中,消化性溃疡组O型血比例高于非O型血(P<0.0125),消化性溃疡组与胃炎组O型血所占比例差异无统计学意义。结论:O型血可能是通过高表达Leb及Hp的babA2增加了消化性溃疡的发生。Objective：To investigate the babA2 of Helicobacter pylori （Hp）and host gastric Lewisb （Leb） antigen status in different ABO blood group,and determine the relationship between the peptic ulcer with it.Methods：Detected ABO blood group of 56 Hp-positive patients （25 with gastritis,31 with peptic ulcer）with peptic ulcer and indigestion.Two gastric specimens got by endoscopy.One was cultured and determined for babA2 genotype by polymerase chain reaction.Gastric expression of Leb obtained from patients with the other one sample was measured immunochemically to determine intensity （range 0-9）.Expression of babA2 and Leb in different blood group,and the relationship with peptic ulcer were compared.Results：Among 56 patients,O-type blood proportion in peptic ulcer was higher than the proportion of gastritis groups and normal groups（P0.0125）,Lebantigen score of O-type blood was significantly higher than no-O blood type（P0.005）.Thirty gastric specimens were cultured successfully and 28 cases were babA2-positive strains（7 with peptic ulcer,21 with gastritis）,among babA2-positive patients with peptic ulcer,O-type blood proportion was higher than no-O blood type（P0.0125）.There was no significant difference between peptic ulcer and gastritis.Conclusion：O blood type increased incidence of peptic ulcer may be through high expression of Leb and blood group antigen binding adhesion of Hp.
