摘 要:目的观察大肠癌患者和接种Colon26细胞小鼠的Th1/Th2平衡偏移情况,同时以IL-21调整小鼠Th1/Th2平衡,观察调节细胞免疫平衡对大肠癌的影响。方法随机选取30例大肠癌患者,采用流式细胞仪测定Th1/Th2细胞比值及血清中IFN-γ和IL-4浓度。另于小鼠爪垫内侧皮下接种高分泌IL-21的Colon26/IL-21细胞,对比观察小鼠肿瘤生长情况及对机体免疫平衡状态的影响。结果①30例大肠癌患者和30例正常人Th1/Th2比值分别为0.74±0.27和1.26±0.9;血清IFN-γ浓度的测定值分别为(6.26±1.43)和(9.18±2.31)pg/ml;IL-4浓度的测定值分别为(8.88±1.30)和(5.20±1.71)pg/ml;IFN-γ/IL-4比值的测定值分别为0.70±0.10和1.82±0.35,两者差异明显。②接种Colon26/IL-21细胞组小鼠肿瘤持续生长至16d后逐渐缩小,小鼠血清中IFN-γ浓度明显高于对照组;接种Colon26细胞组小鼠肿瘤持续生长增大,血清中IL-4浓度高于Co-lon26/IL-21细胞组;免疫平衡向细胞免疫方向转变。结论大肠癌患者与正常人相比细胞免疫功能下降,免疫平衡(Th1/Th2平衡)向Th2型偏移;IL-21可以增强Th1型细胞免疫功能,调节Th1/Th2细胞比例,纠正免疫失衡,有效发挥杀灭肿瘤细胞。Objective To observe the level of Th1/Th2 balance in colorectal carcinoma patient and Colon26 mice transplanted with Colon26 cells in clawpad by flow cytometry and ELISA method;at the same time,the effect of the Thl/Th2 ballance of mice by secreting IL-21 cytokine and how the adjustment of immune balance effect the tumor growth of the mice.Methods 30 cases of colorectal carcinoma patients were selected in.The ratio of Th1/Th2 was detected by flow cytometry.The levels of IFN-γ and IL-4 of the serum were measured.Colon26/IL-21cells were transplanted in clawpad of mice.Results ①The level of Th1/Th2 in colorectal carcinoma patients and healthy adults was 0.74±0.27 and 1.26±0.9;The level of IFN-γ in the serum of colorectal carcinoma patients and healthy adults was (6.26±1.43) and (9.18±2.31)pg/ml;The level of IL-4 in the serum of colorectal carcinoma patients and healthy adults was (8.88±1.30) and (5.20±1.71)pg/ml.The difference of the data between two groups was significant(P0.05).The value of tumor of the mice which was transplanted with Colon26 cells in clawpad decreased gradually and the level of IL-4 in the serum was higher than that of Colon26/IL-21cells.Conclusions Compared with normal immune (balance Th1/Th2 balance) of colorectal carcinoma patients is inclined to Th2 type;IL-21 can enhance the function of Th1 cells and regulate the ratio of Th1/Th2 cells,thus correcting the immune imbalance.Regulating the immune balance can be expected to become a major means of colorectal carcinoma immunotherapy.
