颅内血管网状细胞瘤的MRI诊断与鉴别诊断

摘　　要：目的分析血管母细胞瘤MRI影像学特征,提高血管网状细胞瘤的MRI诊断正确率。方法回顾分析25例经MRI检查及手术病理证实的病例。结果肿瘤位于小脑半球17例,额叶1例,颞叶1例,小脑蚓部2例,肿瘤位于小脑并累及延髓、脊髓、松果体和桥脑小脑角各1例。囊性伴瘤壁结节肿瘤17例,单纯囊性6例,实质性肿瘤1例,混合性肿瘤1例。手术病理,12例囊性肿瘤伴瘤壁结节0.1～2cm;单发者21例,多发者3例,1例颞叶多形性胶质母细胞瘤合并多发性血管网状细胞的混合瘤。结论血管网状细胞瘤的MRI影像学表现具有特征性,分析总结有助于提高对该病诊断的正确率。Objective To analyze the MRI features and differentiation of the intracranial angioreticuloma,improve the diagnostic level of MRI in intracranial angioreticuloma.Methods The plain and post-contrast MRI of 25 patients with intracranial angioreticuloma were analyzed retrospectively.Results The tumor was located in the cerebellum in 17,frontal lobe in 1,temporal lobe in 1,vermis in 2,cerebellum with involvement of medulla,spinal cord,pineal body and cerebropontial angle in 1,respectively.MRI displayed cystic tumor accompanied by tumor wall nodules in 17,cystic tumor in 6,parenchymatous tumor in 1,and mixed tumor in 1.12 were cystic tumors with tumor wall nodules in the size of 0.1～3cm in diameter.The tumor was solitary in 21 cases,and multiple is 3,and the mixed tumor of glioblastoma multiforme and multiple angioreticuloma was located in the temporal lobe in 1 patient.Conclusion The MR fi ndings of intracranial angioreticuloma are characteristic.Analysis of the MRI features conduce to improve the diagnostic level.
