连续性血液净化治疗重症急性胰腺炎的临床观察

摘　　要：目的探讨应用连续性床旁血液净化(CBP)治疗重症急性胰腺炎的临床效果,以有效防止并发症的恶化及减少死亡率的发生。方法回顾性分析2007～2009年我院应用床旁连续性静脉-静脉血液滤过(CVVH)净化方法治疗重症急性胰腺炎或暴发性胰腺炎28例的临床资料。结果重症急性胰腺炎28例,其中暴发性胰腺炎21例。所有患者均在综合治疗的基础上进行CVVH净化治疗,病情好转26例,死亡2例。结论应用CBP治疗重症急性胰腺炎或暴发性胰腺炎,可阻断机体的炎症介质的瀑布式炎症反应,明显的改善了各脏器的功能,病死率显著降低,显示出了其独特的功效。Objective To effectively prevent the complications of severe acute pancreatitis（SAP） and decrease its mortality,we researched the therapeutic action of continuous blood purification（CBP） in SAP.Methods From 2007 to 2009,28 cases of SAP and fulminant pancreatitis were retrospectively analyzed in our hospital,which were treated by continuous vein-vein hemofiltration（CVVH） purification treatment.Results There were 28 cases of SAP,21 of which were fulminant pancreatitis.The patients were administered CVVH,associated with conventional therapy,then,the conditions of 26 patients were improved,2 died.Conclusion CBP shows distinctively therapeutic effect in SAP or fulminant pancreatitis,which could intercept waterfall-type inflammatory response of inflammatory factors,in order to remarkably improve each organ function and decrease case fatality rate.
