同步放化疗联合金龙胶囊治疗中晚期鼻咽鳞癌的临床观察

摘　　要：为了观察金龙胶囊联合同步放化疗(CCRT)治疗中晚期鼻咽鳞癌的临床疗效和毒副反应,将97例中晚期鼻咽鳞癌患者随机分为治疗组(Ⅰ组)49例和对照组(Ⅱ组)48例。Ⅰ组应用金龙胶囊联合CCRT,金龙胶囊4粒/次,3次/d,连服3个月。Ⅱ组仅行CCRT。观察近期疗效、局部皮肤黏膜损伤及治疗前后体质量、Karnosfsky评分变化。治疗组有1例患者因心肌梗死退出,对照组1例因黏膜毒性退出。治疗后、结束后4周及治疗后12周的CR率Ⅰ组均较Ⅱ组高,差异无统计学意义,P>0.05;金龙胶囊明显减轻治疗组的不良反应,对预防皮肤黏膜炎差异有统计学意义。两组患者治疗结束后12周口干发生率差异及治疗结束后Karnosfsky评分差异均无统计学意义,P>0.05。结束后第4周和第12周,Ⅰ组和Ⅱ组患者体质量下降差异有统计学意义(P值分别为0.04和0.023)。初步研究结果提示,金龙胶囊联合CCRT治疗中晚期鼻咽鳞癌临床疗效显著,可显著降低放化疗毒副反应,改善生活质量。This study was to explore the clinical effect and side effect of concurrent chemradiotherapy combined with Jinlong capsules for patients with advanced head and neck squamous cell carcinoma.Totally,97 patients with advanced nasopharygeal squamous carcinoma were randomly divided into combined therapy group（Group Ⅰ） and control group（Group Ⅱ）,49 patients in group Ⅰ and 48 patients in Group Ⅱ.All patients in two groups received radical conventionally fractionated radiotherapy,and received concomitant chemotherapy.Group Ⅰ received Jinlong capsules,4 tablets once,3 times everyday,for 3 months.48 patients were evaluated in Group Ⅰ（one patients quited becaused of myocardial infarction）,and 47 patients were evaluated in Group Ⅱ（one patients quited because of mucosa toxicity）.The complete CR rate of Group Ⅰ was higher than that of group Ⅱ,but two groups were not significantly（P〉0.05）.Jinlong capsules could reduce side effect of concurrent chemradiotherapy.Twelve weeks after treatment,dry mouth,and Karnosfsky were not significantly in both two groups（P〉0.05）.Four weeks and 12 weeks after treatment,the body weight lose of Group Ⅰ and Ⅱ were signrficant（P=0.04,P=0.023）.In conclusion,concurrent chemradiotherapy combined with Jinlong capsules may be an effective and well-tolerated regimen for patients with advanced nasopharyngeal squamous cell carcinoma.Jinlong capsules can alleviate side effect of concurrent chemradiotherapy distinctly.
