蓝染脂质体载体对乳腺癌前哨及腋窝淋巴结显示的应用研究

摘　　要：为了观察局部注射蓝染脂质体在乳腺癌手术中腋窝淋巴结(ALN)及前哨淋巴结(SLN)检测较单用显色剂的优势,利用脂质体-美蓝混合物以及单用美蓝对68例临床Ⅰ、Ⅱ期乳腺癌患者进行前哨淋巴结活检(SLNB)及ALN的清除,均经病理检查,了解两种染色剂在淋巴结染色的灵敏度、准确性方面的差异。脂质体美蓝组36例,成功确定SLN 34例,成功率为94.4%(34/36);SLN阳性16例(47.1%),SLN阴性18例(53%);ALN阳性17例(47.2%),ALN阴性19例(52.8%);SLNB的灵敏度为94.1%,准确性为91.7%。美蓝组32例,成功确定SLN 29例,成功率为90.6%(29/32);SLN阳性11例(37.9%),SLN阴性18例(62.1%);ALN阳性13例(43.8%),ALN阴性18例(56.3%);SLNB的灵敏度为84.6%,准确性为84.4%。两组比较在检查SLNB的灵敏度和准确性方面差异有统计学意义,P<0.05。初步研究结果提示,蓝染脂质体应用于乳腺癌术中,具有淋巴结易着色,可以提高SLNB的成功率、灵敏度及准确性等优点。The objective of this study was to compare the advantages of stain on axillary lymph nodes and sentinel lymph node by local injection of blue-stain liposome with single reagent in breast cancer surgery.The use of liposomes——a mixture of methylene blue and methylene blue alone were used in 68 patients with stage Ⅰ,Ⅱ breast cancer for sentinel lymph node biopsy and axillary lymph node dissection.All patients were characterized by pathological examination,The differences of two kinds of dye staining were compared in sensitivity and accuracy.Liposome group of 36 cases of methylene blue Successfully identified SLN 34 cases.Success rate was 94.4%（34/36）,SLN-positive 16 cases（47.1%）.SLN-negative 18 cases（53%）,ALN-positive 17 cases（47.2%）,ALN-negative in 19 cases（52.8%）,SLNB sensitivity of 94.1%,accuracy 91.7%;Methylene blue group 32 cases,successfully identified SLN 29 cases,success rate was 90.6%（29/32）,SLN-positive 11 cases（37.9%）,SLN-negative 18 cases（62.1%）,ALN-positive 13 cases（43.8%）,ALN-negative 18 cases（56.3%）,SLNB sensitivity of 84.6%,accuracy 84.4%,Comparing the two groups in the examination of the sensitivity and accuracy of SLNB the liposome group was significantly better.In conclusion,blue dye in the liposome used in breast cancer surgery with lymph node is easy coloring,and can increase the success rate,sensitivity and accuracy of SLNB.
