摘  要:目的:探讨Survivin在判断非小细胞肺癌(NSCLC)的病情及预后中的价值。方法:对NSCLC组织50例应用免疫组织化学染色法检测Survivin的表达,并分析其与各项临床因素以及预后的关系。结果:Survivin在NSCLC组织中表达的阳性率为78.0%(39/50);Survivin在NSCLC组织的表达状况与患者的性别、年龄、肿瘤分布、肿瘤直径、组织学分类、分化程度和p-TNM分期无关,P>0.05;生存率的单因素分析显示,Survivin的表达状况(P=0.035)、p-TNM分期(P=0.000)与患者的预后相关;生存率的多因素分析显示,Survivin的表达状况(P=0.027)、p-TNM分期(P=0.000)是影响患者预后的独立指标。结论:NSCLC患者术后生存率受Survivin表达、p-TNM分期等多种因素的影响。Survivin表达是影响患者预后的独立指标。OBJECTIVE:To detect the expression of the Survivin,a novel inhibitor gene of apoptosis,in NSCLC,then to evaluate the clinical significance.METHODS:The expression of Survivin was determined by means of immunohistochemistry in the cancer specimens of the 50 samples.Postoperative survival was univariately analyzed by the Kaplan-Meier method,and the difference was assessed by the Log-rank test.Multivariate analyses were performed by using the COX proportional hazards modeling.RESULTS:The expression rates of Survivin in NSCLC tissues was 78.0%(39/50).Univariate analyses demonstrated that the survival rate of the 50 NSCLC patients was significantly affected by Survivin status (P=0.035) and p-TNM stages (P=0.000),and not by sex,age,tumor distribution,diameter,histology type,differenition and p-TNM staging(P〉0.05).Multivariate analyses (Cox proportional hazards modeling) revealed that Survivin status (P=0.027) and p-TNM stages (P=0.000) were independent prognostic factors of NSCLC patients.CONCLUSIONS:The survival rate of NSCLC patients is affected by such prognostic factors as Survivin status and p-TNM stages.Survivin status,p-TNM stagesare the two independent predictors of NSCLC patients.
