子宫内膜样腺癌组织OPN和CD44V6表达临床意义的研究

摘　　要：目的:探讨OPN、CD44V6抗原的表达与子宫内膜样腺癌临床病理特征的关系。方法:应用免疫组化SP法对60例子宫内膜样腺癌标本进行OPN和CD44V6抗原检测。结果:OPN在增生性子宫内膜、分泌性子宫内膜及子宫内膜样腺癌中的表达率依次升高,组间差异有统计学意义,P<0.01;OPN在子宫内膜样腺癌Ⅰ～Ⅱ期和Ⅲ～Ⅳ期中的表达率分别为70.59%和92.31%,差异有统计学意义,P<0.05;在转移组中OPN的表达率为95.00%,无转移组表达率为72.50%,转移组的表达率高于无转移组,P<0.05。CD44V6在子宫内膜样腺癌中的表达高于增生性子宫内膜和分泌性子宫内膜,P<0.05;CD44V6在子宫内膜样腺癌Ⅰ～Ⅱ期和Ⅲ～Ⅳ期中的表达率分别为52.63%和81.82%,分期越高表达率越高,差异有统计学意义,P<0.05;CD44V6转移组表达率为85.00%,无转移组组表达率为52.50%,转移组的表达率高于非转移组,P<0.05;肌层浸润>1/2组表达率为85.71%,肌层浸润≤1/2组表达率为51.28%,随浸润深度的增加表达率升高,P<0.05。结论:OPN、CD44V6高表达提示子宫内膜样腺癌生物学行为不良,联合检测OPN、CD44V6可作为判断子宫内膜样腺癌恶性程度、预测转移和评估预后的生物学指标。OBJECTIVE：To study the relationship between the expressions of OPN and CD44V6 antigens and the chinical and pathologyical characteristic of endometrial carcinoma.METHODS：The expressions of OPN and CD44V6 were detected in 60 cases of endometrial cancer specimens with the immunohistochemical SP method.RESULTS：The expression level of OPN increased progressively from the proliferative stage endometrium tissue and secretory stage endometrium tissue to endometrioid adenocarcinoma tissue（P0.01）.There was a relationship between the expression of OPN with the clinical stage（70.59% in Ⅰ-Ⅱ stage,92.31% in Ⅲ-Ⅳ stage）and lymph node/extra-uterus metastasis（95.00% vs 72.50%,P0.05）.The positive rate was higher in the endometrioid adenocarcinoma tissue than that in the proliferative stage endometrium tissue and secretory stage endometrium tissue（P0.05）.There was a relationship between the expression of CD44V6 with the clinical stage（52.63% in Ⅰ-Ⅱ stage,81.82% in Ⅲ-Ⅳ stage）,muscle invasive depth（85.71% in group invasing 1/2 muscular coat,51.28% in group invasing ≤1/2 muscular coat）and lymph node/extra-uterus metastasis（85.00% vs 52.50%,P0.05）.CONCLUSION：The detection of OPN and CD44V6 in combination may be a biological index in judging the malignancy,predicting the metastasis and evaluating the prognosis of endometrial carcinoma.
